Although I don't see how this could be map specific, this is the only map its happened on with ~500 hours of gameplay

-Added a document to Sanctuary that provides information about the difficulty of various areas. It will be automatically added to your character when you first enter the starting zone.
-Fixed the “Slay the Seven” quest to not show as completed when Khaal dies. You must kill Ur to complete it.
-Added an Ordeal to the Temple of Ordeals: “The Ordeal of Heaven.”
-Added legendary pikes, halberds, longmaces, and hand crossbows to the Master Dwarven Smith’s store. Decreased weapon prices from 300,000 to 200,000.
-Changed the teleporter structure in the wizard towers. They are now endurance challenges.
