I don't know how to port for SFM, nor I have it, sorry :/
Ask a guy named Ageha on Facepunch he's into porting Dark souls models into SFM or other named Skyscream Sam here.

It was not meant for SFM you can see it pretty well at side of the page, you utilize it in Garry's Mod.
