If you are playing in Multiplayer, having this as a mod on the server isn't enough. This needs to be enabled in your Zomboid game's mod screen from the main menu, as it directly impacts your UI. Generally, anything that changes the UI or (if you're hosting) adds sandbox settings to your game is a good idea to enabled from the main menu to ensure it's loaded and working, despite the server's modlist. (I don't often play singleplayer so I can't speak for any issues with it there.)
