This guide will detail useful information on how to obtain all 50 Fallout 4 achievements.

Down the right side, you can find various achievement names and can easily find the achievement you're looking for. Any spoilers will be in spoiler text. The guide will be structured into Unmissable story, Factions and then the other achievements separate below that. (Some will be grouped like the achievements unlocked for leveling up to specific levels e.g. Born Survivor, Commonwealth Citizen etc.)

Unmissable Story Achievements
These are the achievements that you will get through the main story with any faction:

-War Never Changes:
"Enter The Wasteland."
Achieved after exiting Vault 101.

-When Freedom Calls:
"Complete "When Freedom Calls""
Achieved after completing an early story quest, When Freedom Calls.

-Unlikely Valentine:
"Complete "Unlikely Valentine""
Achieved after completing a story quest in Diamond City, Unlikely Valentine.

-Reunions:
"Complete "Reunions""
Achieved after completing act one of the story.

-Dangerous Minds:
"Complete "Dangerous Minds""
Achieved after doing the story mission at the Memory Den.

-Hunter/Hunted:
"Complete "Hunter/Hunted""
Achieved after killing a courser and finishing the quest.

-The Molecular Level:
"Complete "The Molecular Level""
Achieved after doing the story mission and accessing the Institute for the first time.

-Prepared For The Future:
"Decide the Fate of the Commonwealth"
Achieved for completing the games story with any faction.
The Nuclear Option
-The Nuclear Option:
"Complete "The Nuclear Option""


This quest will be led to by all factions other than the Institute. Following either the Railroad, Brotherhood of Steel, or the Minutemen, through to the end of the game will allow you to obtain this achievement as part of the main story. However, if you play with the Institute you will not unlock this achievement.
Institute Faction Achievements
-Institutionalized:
"Complete "Institutionalized""


You will unlock this quest after entering the Institute and can be completed without angering any faction.
-Mankind-Redefined:
"Complete "Mankind-Redefined""


This mission will come later in the Institute faction story. It is possible to comeplete this quest without angering any faction.
-Powering Up:
"Complete "Powering Up""


This quest is after Mass Fusion and Pinned. You will have already made at least one faction hostile (Brotherhood of Steel).
-Nuclear Family:
"Complete "Nuclear Family""


This is achieved after finishing the game with the Institute and will be awarded after completing the short quest Nuclear Family.
Minutemen Faction Achievements
All Minutemen faction achievements can be achieved without turning any faction hostile if you stop their quests after finishing Old Guns.

-The First Step:
"Join the Minutemen"


This achievement will be obtained upon completing the quest The First Step.
-Sanctuary:
"Complete "Sanctuary""


This quest is obtained after helping the group you rescue in When Freedom Calls and move them to Sanctuary. The quest is bascily a tutorial for base building.
-Taking Independence:
"Complete "Taking Independence""


This quest comes later in the story for the Minutemen, and involves taking back the Castle from Mirelurks.
-Old Guns:
"Complete "Old Guns""


This is a later Minutemen quest that is unlocked later at the Castle.
Brotherhood of Steel Faction Achievements
-Semper Invicta:
"Join the Brotherhood of Steel"


This achievement will be obtained upon completing the quest Semper Invicta, by returning to the Cambridge police station and talking to Paladin Danse. This quest can be completed without making other factions hostile.
-Blind Betrayal:
"Complete "Blind Betrayal""


This quest will come later in the Brotherhood of Steel quest line. It is likely you will have made some factions hostile at this point.
-Ad Victoriam:
"Complete "Ad Victoriam""


This is the final Brotherhood of Steel quest before the Nuclear Option.
Railroad Faction Achievements
-Tradecraft:
"Join the Railroad"


Complete the quest, Road to Freedom and then the followup quest, Tradecraft and you will get the achievement. This will make no factions hostile.
-Underground Undercover:
"Complete "Underground Undercover""


This quest is a lengthy quest and involves saving synths. Complete it for the achievement. Should start after siding with the Railroad in the Molecular level and finishing Tradecraft.
-Rockets' Red Glare:
"Complete "Rockets' Red Glare""


This is the final Railroad quest before the Nuclear Option and will involve taking down the Brotherhood of Steel.
Levels
These achievements are just for leveling up and are self-explanatory:

-Born Survivor:
"Reach Level 5"


-Commonwealth Citizen:
"Reach Level 10"


-Unstoppable Wanderer:
"Reach Level 25"


-Legend of the Wastes:
"Reach Level 50"
Community Organizer
-Community Organizer:
"Ally with 3 Settlements"


This achievement can be obtained by working with the Minutemen early on in the game and by completing radiant settlement quests given by Preston. You can quickly and easily gain 3 allied settlements this way.

Some examples of such quests:
  • Kidnapping quests
  • Ghoul problem quests
  • Raider problem quests
  • Taking point quests

Examples of settlements:
  • Abernathy farm
  • Graygarden
  • Greentop Nursery
  • Oberland Station
  • Starlight Drive-In
  • Sunshine Tidings Co-op
Benevolent Leader
-Benevolent Leader:
"Reach Maximum Happiness in a Large Settlement"


This achievement will be the most time-consuming. You have to use the workshop to make one of your settlements reach the max of 100 Happiness. Here is some general information below when going for this achievement:
  • The achievement can be unlocked with any number of settlers in any settlement. Graygarden is not recommended as the happiness of Mister Handy settlers is limited to 50%.

  • The size of the settlement refers to the number of structures in the settlement, not the number of settlers. However, in spite of the description, the size of the settlement does not have to be large (i.e. yellow/orange colour).

  • The happiness cap without any bonus happiness is around 80, which should be reached by providing enough water, food, beds (covered) and defense.

  • To get additional 20 happiness, one can use stores. Junkyard dogs, cats and gorillas, or slot machines, soda fountains, phoropters, weight benches and pommel horses from the Vault-Tec Workshop add-on.

  • Synth settlers and noise level have no effect on happiness.

  • Local leader level 2 perk will be needed.

Gun-For-Hire

Side quests can be found all over the Commonwealth. With a total of 35 side quests, there is plenty to find. Note that you can track how many completed side quests you have from your pip boy data and that faction and main story quests do not count including faction radiant quests.

Mercenary
-Mercenary:
"Complete 50 Misc. Objectives."


There are actually very few Misc objectives in Fallout 4 and they can prove difficult to find. Firstly, here's a link to the non-repeating ones that you can have a look at if you need a few more.

I highly recommend doing the Atom Cats misc objectives as that will give you about 7 of them.

However, if you are struggling you can farm misc objectives. Firstly you have to complete Brotherhood of Steel quests until you unlock Vertibird Signal grenades. After you receive some for completing a quest you can purchase them aboard the Prydwen from the shop.
You can then throw these grenades in the Commonwealth and a Vertibird will come which when landed will give you a misc objective to board it. You can then board and unboard and fast travel and do this again until you have done a total of 50 misc objectives.

Scavver
-Scavver:
"Gather 1000 Resources Used For Crafting"


For this achievement you will need to gather 1000 crafting resources. These include:
  • Aluminum
  • Adhesive
  • Ceramic
  • Copper
  • Glass
  • Nuclear material
  • Plastic
  • Wood
  • Steel etc.

Emptying a very full workbench, then refilling it, and unloading again will also count towards gathering these resources so can be done very quickly.
What's Yours Is Mine
-What's Yours Is Mine:
"Pick 50 Locks"


This achievement is very simple and you will most likely get this throughout the course of the game. If not just explore until you find locked doors or containers because there are plenty of them.

RobCo's Worst Nightmare
-RobCo's Worst Nightmare:
"Hack 50 Terminals"


This achievement, like What's Yours Is Mine, is very simple. Hacked terminals can be found by exploring. If you are struggling, try looking for factories and warehouses as that's where there most prominent.

Armed and Dangerous
-Armed and Dangerous:
"Create 50 Weapon Mods"


This achievement can come easily to those who get into the weapon modding.

If you are struggling you can get a baseball bat and use wood to change the colour of the bat in the modding. Every new colour counts as one mod. After getting all of them you can go to your inventory and drop the mods then remod the bat to all the colours again until you have 50. This is a cheap and easy method if you haven't modded much.

Each mod you build for one weapon counts for this achievement. Which means you can get a regular pipe pistol and create 25 mods for it, and all 25 of those mods count towards this achievement. You are not required to mod 50 different weapons.

To view your progress toward this achievement - open up your PipBoy and go to DATA, STATS, Crafting.

Wasteland D.I.Y.
-Wasteland D.I.Y.:
"Craft 100 Items"


This achievement is also another fairly simple one. Cooking food and crafting chems count toward this, but mods and workshop items don't count. Fertilizer and plastic make Jet which can make this go extremely fast. Combining drugs (i.e. Buffjet, Bufftats, Psycho Jet, etc.) will also count towards this achievement.

Lots of fertilizer can be found at Graygarden, and I would recommend picking up meat from all the animals you kill to cook.
Never Go It Alone
-Never Go It Alone:
"Recruit 5 Separate Companions"


For this achievement, 5 companions need to be recruited with the same character. There are 13 companions in Fallout 4 without the DLC.

Companions:
  • Cait - Ask her to join after clearing the Combat Zone
  • Codsworth - Ask him to join after visiting Concord in the quest Out of Time
  • Curie - Complete the quest Hole in the Wall (Vault 81)
  • Paladin Danse - Upon receiving the quest Tour of Duty (Brotherhood of Steel quests)
  • Deacon - Complete Tradecraft and join The Railroad
  • Dogmeat - Ask him to join after visiting the Red Rocket truck stop
  • John Hancock - Ask him to join after completing The Big Dig (if sided with Bobbi No-Nose, after completing Recruiting Hancock)
  • Robert Joseph MacCready - Hire him for 250 caps or 200 (Easy speech check)
  • Nick Valentine - Complete Getting a Clue (Main story)
  • Piper Wright - Complete Story of the Century (side quest given after entering Diamond City
  • Preston Garvey - Complete The First Step and join the Minutemen
  • Strong - Ask him to join after Curtain Call
  • X6-88 - Join the Institute and complete Mankind - Redefined

Recruit any 5 and the achievement will be obtained.
Lovable
-Lovable:
"Reach Maximum Relationship Level with a Companion"


For this achievement you need to recruit a companion and when they are following you certain actions or dialogue choices will show a little box in the top left corner and it will say something like the following:
  • 'X' liked that.
  • 'X' loved that.
  • 'X' disliked that.
  • 'X' hated that.
    'X' = companion name.
As you can gather, liked and loved are what you want to reach the max level of affinity with your companion. You can treat this as a like will cancel out a dislike and so on. Don't worry as it is hard to get your companion to hate you. Different companions are easier than others and they all like and dislike different things.

For example, Piper likes lockpicking unowned doors and locks and Codsworth loves crafting weapon mods. Nick likes hacking terminals and Cait likes it when you don't wear clothes. They are all quite varied.

People say the easiest person to get to max affinity is Codsworth because of his love for crafting weapon mods. However, if you stick to one companion you can easily get any of them* to max affinity across a playthrough.

*except for Strong. He dislikes almost everything the player does.

Paladin Danse, Curie, Cait, and Nick Valentine cannot reach max affinity until their respective personal quests have been completed.
Fix-Er-Upper
-Fix-Er-Upper:
"Build 100 Workshop Items"


Simply, have a friendly settlement and open the workshop to place items. You can track how many workshop items have been built from the pipboy. Make sure to collect lots of scrap.

Wooden crates are probably the easiest to craft cause they only require 3 wood per crate. They can be found under, Furniture → Containers in the workshop.
Future Retro
-Future Retro:
"Play a Holotape Game"


Holotape games can be found in various comic books and magazines across the Commonwealth. Once found they will appear as holotapes in your inventory and can be played just like any other holotape from the 'misc' page of your pipboy screen.

Holotape games and their location:
  • Atomic Command - It can be found at the top floor of the Museum of Freedom beside the active terminal.

  • Grognak & the Ruby Ruins - The tape can be found paired with an issue of RobCo Fun in the basement of the Memory Den in Goodneighbor.

  • Pipfall - It can be found at the Fort Hagen Command Center, next to Kellogg's terminal, under a table.

  • Red Menace - The game can be found on the Vault 111 recreational terminal in the cafeteria.

  • Zeta Invaders - It can be found on a desk, inside the Valentine Detective Agency located in Diamond City Market.

Once you have one just load it up in your pipboy for the achievement.
…The Harder They Fall
-…The Harder They Fall:
"Kill 5 Giant Creatures"


The only two enemies that count for this trophy are super mutant behemoths and mirelurk queens. You have to kill 5 of any of these creatures, in one playthrough for the achievement.

Below is the locations:

Super mutant behemoths:
  • Outside of Fort Strong.
  • West of Natick Banks.
  • In the woods south of Greentop Nursery and north of Malden Middle School. Only spawns after removing a Jangles the Moon Monkey toy from a car crashed into a tree.
  • West of the Suffolk County charter school.
  • South of Walden Pond at a car-like Stonehenge.
  • Swan is a unique super mutant behemoth and can be found at Swan's Pond at Boston Common.
  • South of Recon bunker Theta.
  • Random spawn in front of the Super Duper Mart in Lexington.
  • During the quest Ad Victoriam, one will spawn on the way to C.I.T. ruins.

Mirelurk queens:
  • At the Castle as part of the quest Taking Independence.
  • At the Murkwater construction site, must be defeated in order to use the area for settlement.
  • Spawns at Spectacle Island once throwing the circuit breaker.
  • At Nahant Chapel, nesting near the coast.
  • In the basement of the Massachusetts State House.
  • South of Warwick homestead and east of the Poseidon Energy plant, close to a capsized boat in the swamp.
  • To the west of Salem, northwest of the Museum of Witchcraft.
  • By the beach just southeast of Revere Beach station.
  • Southeast of Recon bunker Theta.
Ranger Corps
-Ranger Corps:
"Discover 100 Locations"


Just explore the Commonwealth. Finding 100 locations isn't hard and doesn't take long either.

Print's Not Dead
-Print's Not Dead:
"Read 20 Magazines"


There are over 120 magazines in Fallout 4, excluding DLC ones. (Yes I added them up) so I'm not going to compile a list on this guide. They pop up in lots of locations and you shouldn't have too much trouble finding 20 over a playthrough.

You can check how many you have collected from your pipboy data screen.
Prankster's Return
-Prankster's Return:
"Placed A Grenade Or Mine While Pickpocketing"


For this achievement, you will need to acquire Pickpocket level 2 as the grenade or mine has to detonate for the achievement to pop. The requirements for Pickpocket level 2 is Perception 1 and being level 6 as well as the 2 skill points that need to be spent.

This achievement doesn't have to be gotten on enemies and can be gotten on NPC's in Diamond City etc.

Masshole
-Masshole:
"Kill 300 People"


For this achievement simply blow 300 humans to bits. You can track how many people you have killed in the pipboy data screen. This should unlock over the course of a playthrough.

Enemies that count are:
  • Raiders
  • Gunners
  • Any settlers you kill etc.

Animal Control
-Animal Control:
"Kill 300 Creatures"


This achievement is very similar to Masshole but instead of killing people you need to kill any of the following:
  • Feral ghouls and ghouls
  • Super mutants - hounds and normal ones
  • Mole rats
  • Radscorpions
  • Bloatflies
  • Deathclaws
  • Bloodbugs etc.

Should unlock over the game.
Homerun!
-Homerun!:
"Get a Homerun"


For this achievement, you need to go to Diamond City and go from the first base to the second then third and then back to the first just like baseball. The plates are numbered with green Roman numerals and can be easily found on the floor.

To start, search the ground between Publick Occurrences and All Faiths Chapel for the home plate. Run to your right, past the Dugout Inn, the first plate should be on the steel walkway that turns left to the farm. Keep running to next base which is placed on the wood planks floating in a puddle entering the farms. Run to your left again down the alley, past the Valentine Detective Agency, the base should be at the intersection at the end. Run past the Security office and back to homeplate.
Touchdown!
-Touchdown!:
"Get a Touchdown"


For this achievement, you have to be killed by any nuclear explosion in the game. The easiest way is by a super mutant suicider's explosion, but you can also be killed by a Fat Man in order to get this achievement.

This achievement can also sometimes occur when your companion is 'kneeled' by a nuclear explosion.
They're Not Dolls… …They're Action Figures
-They're Not Dolls…:
"Collect 10 Vault-Tec Bobbleheads"

"Collect 20 Vault-Tec Bobbleheads"


These two achievements are awarded for collecting all 20 of the collectible Bobbleheads from across the Commonwealth. Their locations are listed below as well as a YouTube video if you want a visual guide.

Locations:
  • Strength bobblehead - Mass Fusion building. On the metal wall sculpture high above the lobby desk.

  • Perception bobblehead - Museum of Freedom. On a desk next to a broken terminal in the back of the room where the player character first meets Preston Garvey.

  • Endurance bobblehead - Poseidon Energy. On the metal desk with a magazine, near steamer trunk, central metal catwalk.

  • Charisma bobblehead - Parsons State Insane Asylum. On Jack Cabot's office desk, close to the elevator, administration area.

  • Intelligence bobblehead - Boston Public Library. On the computer bank, mechanical room, northwest corner of the library.

  • Agility bobblehead - Wreck of the FMS Northern Star. On the edge of the bow of the ship, wooden platform.

  • Luck bobblehead - Spectacle Island. In the 2nd deck pilot house of a green tugboat located at the southern end of Spectacle Island, on a locker shelf near the steamer trunk.

  • Barter bobblehead - Longneck Lukowski's Cannery. Inside the metal catwalk hut, southwest upper area of the main cannery room.

  • Lock picking bobblehead - Pickman Gallery. Last tunnel chamber where one can see Pickman; On the ground between brick pillars and a bin fireplace.

  • Medicine bobblehead - Vault 81. In Curie's office, southeast corner of the vault.

  • Repair bobblehead - Corvega assembly plant. On the very end of the top exterior gantry (blue balloon), southwest roof section of the plant building.

  • Science bobblehead - Malden Middle School (Vault 75). On the desk overlooking the subterranean "training" area, within the science labs.

  • Sneak bobblehead - Dunwich Borers. On a small metal table by a lantern, right next to the metal post terminal area #4.

  • Speech bobblehead - Park Street station (Vault 114). In the overseer's office.

  • Big guns bobblehead - Vault 95. In the living quarters area, northmost room, on a radio.

  • Energy weapons bobblehead - Fort Hagen. In the command center, southwest kitchens, on a small table between two fridges. (Accessible only during/after the main quest Reunions)

  • Explosives bobblehead - Saugus Ironworks. In the blast furnace on the second level catwalk behind Slag's spawn and next to a steamer trunk.

  • Melee bobblehead - Trinity Tower. On a table in the cage where Rex Goodman and Strong are being held.

  • Small guns bobblehead - Gunners plaza. On the broadcast desk in the on-air room, first floor, west side of the building.

  • Unarmed bobblehead - Atom Cats garage. On the hood of the rusty car in the main warehouse.

@ZachTempo, I am afraid I do not know the answer to that question. I believe there is a way to track stats in the pip-boy for hacked terminals so might be worth testing with that.

Excellent and extremely useful guide. I only have two Railroad Faction Achievements remaining at the time of this writing and I know I'll get them in due time. This guide was particularly helpful in getting the "Benevolent Leader" Achievement, as I didn't even attempt it until late in my original playthrough and I didn't want to start a new game just for this, so this guide gave me everything I needed to make it happen. Thanks for putting this together and stay safe out there!

Excellent guide! Very helpful.

I have a few extra tips for "benevolent leader" - follow the above tips AND also get a cat and a dog using the workshop DLC.

Also don't have ghouls or synths in your settlement.
I had Hancock and Curie in Red Rocket and I got maxed out at 94. I sent them elsewhere and I jumped up to 98. Then I crafted a quitting time siren and waiting another 2 days got me there.
