The PC version of this game is infamously known as one of the worst optimized ports of all time, right behind Saints Row 2 and Batman: Arkham Knight. Though, I was able to solve my performance issues by using this:

GTA 4 and it's single-player DLCs are classic Rockstar Games masterpieces and I definitely recommend it to anyone who hasn't (somehow) played them yet.
