If switching mods fixes that problem I'll return to let y'all know.
As for the "strange healthcare tab", since this mod destroys and recreates the Healthcare menu to separate Healthcare from Eldercare there was some sort of tab that showed up. The tab comes from vanilla code which I assume doesn't show in most, if not all, cases. Just in case that tab is for some DLC or content pack or something, I didn't delete it outright and instead opted to hide it based on a menu option.
When this mod is active, there is an extra tab/label/sub-menu under the Healthcare tab/menu.
... it has a lot lower Mesh Count than the PEC, and this one doesn't require any DLCs either.
Sometimes the problems we have with the game aren't in mods, but in simple assets. For example, ever since the Green Cities release, the game locks-up almost immediately on my PC (thus, I play with the pre-GC version). The problem I have isn't due to any the mods I use, but rather to some plain old residential assets.
The crash made by random clicked some service building. (This time I clicked Public Library)
So, I feel mod need to rewirte with Harmony 2 will be good more.
And yeah, the mod works fine with Sunset Harbor, so support shouldn't be an issue, as I believe that is the final DLC for CS.
So, finaly I found incompatiable point between those two mods.

So, if mod itself can got option to adjust, it will no need "Customize It Extended" to do.
And also good, if "Customize It Extended" can supported this mod.
As far as I know, that'll do the trick, and it's quite straightforward to do as well. And hopefully, you've got multiple previous saves to fall back on to rescue your city!

A 2 Star Nursing Home has a -1, 0.5, and 0.5 chance multipliers for high, medium, and low wealth Cims respectively. So moderate chance for medium and low wealth, low chance for high wealth.
A 1 Star Nursing Home skews even further towards low wealth Cims with a -2, -0.25, and 1 chance multipliers for high, medium and low wealth Cims respectively.
But then for a 0 Star, it goes back the other direction a bit due to the idea of forced residence: -0.5, 0.25, and 2 chance multipliers for high, medium, and low wealth Cims respectively.
Also added an additional image to demonstrate that the Asset functions in locations remote from the city, and also to incorporate ploppable Watchtowers. Plus I added a couple of extra tips on how to optimise traffic vs number of occupants.
I wasn't sure what Level 0 would do, so I left it at the default of 2. But from what you say, I reckon Level 0 would be better-suited, so I'll update it later today. And the baseline number of beds is 250 - the 375 in the example was due to the 1.5 multiplier.
And even though I placed my PEC in the depths of a forest remote from the city, it became well patronised pretty quickly!

Sometimes assets themselves cause extra tabs to appear for some menu panels.
For example, I have two parking garage assets which caused a "Road Conditions" tab to appear on the Roads panel. I select/plop them from there, because they don't appear on any Parks tab, even though they are parks.
As another example, I have some RICO farm assets (parks without RICO) which caused a "Plazas" tab to appear on the Healthcare panel. They also don't appear on any Parks tab, but since they are RICO enabled, I plop them from that panel.
Perhaps it's the "More Advanced Toolbar" mod which is causing the issue you see.
The last time I personally tested and verified this mod was the Park Life DLC. Others reported no issues up til the Campus DLC so I marked compatible through that, but I no longer support this mod. There's still the odd comment that it works great and other odd comments that people wish this would be updated, so I'm not really sure how well this still works, or how well it works with various DLC/Mod combinations.
People tried to find someone to take over this mod to no avail, but that option is still open for anyone who wants to take owneship and support this going forward. There's even the possibility of using this same idea to create Dorms for Teens going to college.
