How to link your Smartphone to Fallout 4 (Android, iOS, and Windows Phone)
By Akarinata
A quick guide for those unable to connect their device to Fallout 4. Note that this requires a broadband internet connection, and for both devices to be connected on the same network, but this should be of no surprise or obstacle to the average Steam user.

Step 1: Open Fallout 4, and turn "Pip-Boy App Enabled" to ON.
This option is in the Gameplay Options menu, at the very bottom.

Step 2: Restart Fallout 4.
You'll need to load up your save file before the next step as well, as the app won't be able to connect before you do so. You may pause the game after you have loaded your file, as the app with connect regardless of whether the game is paused or not.
Step 3: *Crucial* Allow Fallout 4 to use your PC's network connection.
This is crucial, and I suspect the No.1 reason many people are having so many issues pairing their app to the game. A pop-up should appear on your desktop asking to give Fallout 4 permission to use your network, thereby exempting it from Windows Firewall. However, it does not interrupt Fullscreen mode, which I assume many people are using as well as I, so you would normally never receive any indication that Fallout 4 is asking for permission to use the network. Simply Alt-Tab to check your desktop for the notification at this point, and the pop-up should be flashing on your taskbar to indicate Fallout 4 is asking for permission for a Firewall exemption. Obviously, this is where you click "Allow". Once you have done so, return to Fallout 4.
Step 4: Open the Official Pip-Boy Companion app on your Android or iDevice.
If you have not done so already, download the official Pip-Boy companion app to your smartphone from either the App Store, Windows Store, or Google Play.
Once you have downloaded the app, open it, and choose "PC" (duh) on your device, and the app will attempt to connect to your device. It should be noted that both your smartphone and your PC need to be connected on the same network to sync, but it does not matter whether your PC is hardlined to your router, or whether it is wirelessly connected.
Step 5: Connect to your PC from the Companion App.
Another simple step. From here, all you do is connect to your PC's local network address (something along the lines of 192.168.x.x) by tapping on the address itself.
If you have succeeded, this is what you should see, followed by your Pip-Boy's realtime stats:
End Guide: Enjoy!
Enjoy the newest entry in "immersive" vidya gimmick shenanigans!

Sorry for any errors in formatting or lack of abundant content, as this was my first-ever guide, but I had lots of fun making it, and I'll most likely make more useful guides in the future. Thanks for reading!

@Festive DogePhonex: Yes tried that several times. Just have to keep on trying :). I will try to create a new character and see if that works.

@ Dr.Feelgood: Might be a bug, or a problem with your actual connection. Have you tried restarting both the game and the app?

Done all the steps and gets the IP from my computer. When I then click the IP I first get Connection Success but directly after it says Connection Error.
Got Win 7, tried on both Galaxy S6 and Sony Experia Z3.
Any thoughts?

@alexm.box You can play games from the interactive holotapes you've collected, I believe you can play music from the radio stations, but other than that, it's kind of a gimmick, truth be told. Not that I advertised it as anything else.

Does the app allows anything besides viewing your inventory?

Works even for pirates. Router splitting cable for PC and wifi for a standard android phone. Enabled F4 to bypass firewall. Worked right out of the bag.
