Don't Copy That Floppy
Welcome to the guide to help decipher your floppy before the Operation Chaos side mission.

If you used this guide, I would appreciate a thumbs up and/or a favorite. It keeps me writing these.

Now let's get back to the guide!

This answer is different for everyone, so there is a VERY SMALL chance that yours will be the same as mine. Even the information on my screenshots will be different.

If you want to try anyway before actually reading this on the chance that you managed to get the same as me copy the credentials I used.

If you were ♥♥♥♥ out of luck, see below and time to start using your head in your Call of Duty game and make sure you have a notepad open on your second monitor or a pen and paper to remember all your answers.
Where to Find the Evidence
First you will need to find each of the evidence. Operation Chaos will unlock early before you even manage to get all the required evidence needed. You can start it at that time but if you want the best ending then it's best to decipher the floppy disk.

In Nowhere Left to Run when you interrogate Qasim do not throw him off the building right away. Keep talking to him until he gives you the evidence.

In Brick in the Wall follow the optional mission marker to rescue Richter. You'll have to pick a lock, go down a basement and kill some guards. Grab the evidence on a desk in front of Richter.

In Redlight, Greenlight when you reach the American Town go into the bar near the 60 min photo store and head upstairs. In an office you'll see a map. Just take out your camera and take a picture of it.
Newspaper

On the first evidence with the newspaper you will see a bunch of letters in red. Mine were I-H-E-L-G-A-R. This is an ANAGRAM so you will need to scramble those letters until you get a city name that matches the list of cities on the 3rd evidence. For example, mine spelled out Raleigh.

Remember your city name for later.
Coded Message

For this evidence you will need to separate the red numbers and blue numbers to figure out the missing two numbers.

So for example mine was
Red: 17 - 18 - 20 - ? - 27 Blue: 33 - 37 - 41 - ? - 49

I added up the difference between each number and you'll see my RED numbers added 1 then 2 between the first 3 numbers. So then i'll add 3 to 20 and get 23. To confirm I was right, I add 4 to 23 and it will equal 27.
So it was 17+1 = 18, 18+2 = 20, 20+3 = 23, 23+4 = 27.

Same goes for BLUE, but this was much simpler for me. Each number was only 4 numbers apart between the first 3 so I added 4 to 41 and got 45. To confirm I was right, 45+4 = 49.
So it was 33+4 = 37, 37+4 = 41, 41+4 = 45, 45+4 = 49.

Take those two ? numbers and keep them to the side. So my numbers are 23/45.
Numbers Station Broadcast

So now that I have my city name, and numbers coded. Take the city name you got and that is your first line of code.

My Newspaper article spelled out Raleigh so that makes my first line 4490.

Now grab the code you got from the 2nd evidence and you'll get your key phrase from this same list.

My code was 2345 so that makes my answer Las Vegas.
Access Granted

You'll enter the codes in like above. Inputting the numbers from the first evidence that matched up with the broadcast and then inputting the CITY NAME from the 2nd evidence into the Passphrase.

For me the letters in the newspaper headline didn't turn red until I went back to it. I was looking at that page trying to see a pattern for ages! Is there a way to work out the letters before the game highlights them?

Thank you~ You managed to set this up so I only read what I needed, and I'm impressed with Treyarcs ability to create a puzzle satisfying to solve like this.

This makes me feel insane, and I love the list of numbers and places I have scribbled. You're a life saver, have a good day!

I figured it out, there are 6 individual pieces of evidence, mostly maps, that you gotta take pictures of in this mission to get the evidence.
