i heard that this problem had something to with the Tunnel Snakes CC . when i had to re-install FO4 on a new computer. i didn't re-install the tunnel snakes CC. and henceforth never had this issue of not being able to leave the vault again.

some people mod it from the start

its called not being smart enough yo get the script plugins that allow you to get them while playing fully modded games and also those that dont read the mods pages almost all that add large scripts to the start say after picking up the pipboy to wait upto 5 or ten mins before using the door control a way to tell if this will happen on your game is in the front room of the vault if the left wall vanishes from where you get the pipboy and are facing back into the vault then wait
