Since its just png textures and an XML
My final tip would be to remove the mod and check if the original missing eye texture appears without it. If that doesn't work move my mod to the end of the modlist and try again. If none of these gives any results then you'd have to search your modlist for anything that might be messing with wound textures
- The bionic eye generates textures
- The advanced bionic eye doesn't generate textures
- The archotech eye generates textures
- And of course no eye generates the base game eye texture added by ideology.
Test the other prosthetic eyes to see if they still generate textures. This is definitely caused by another mod.
Do you have any mods installed that remove wound rendering by any chance?
Perhaps you could tie it in with EPOE's eyepatch prosthetic? If not then you can go with CrackaJack's suggestion of making it a new prosthetic.

on a graphical level, probably not, since that mod might have textures where the eyes are placed too far to the side to be covered by the eyepatch
