Finishing the main game on Death March difficulty will grant you this achievement and the two previous ones.

On this difficulty, the first 7 levels might prove to be quite hard but after some grinding (and also getting used to the game's mechanics) you shouldn't struggle with quests that are not more than 5 levels above your character level.
Basically the higher the difficulty, the more you will need to grind.


Note : Lowering the difficulty will void the achievement
If you try to lower the difficulty, the game will give a pop up the first time to make sure that's what you want to do.

You must complete the entire subplot involving Keira Metz BEFORE The Isle of Mists.

You must complete either Possession or The Lord of Undvik secondary quests and then complete King’s Gambit secondary quest BEFORE Battle Preparations.

A Matter of Life and Death, Now or Never, Redania's Most Wanted, A Deadly Plot, An Eye for an Eye must be completed BEFORE The Isle of Mists.

Then during the main quest Blindingly Obvious, when Dijkstra blocks your path, you must CHOOSE totell him Emhyr's plan.
Finally you must now participate in Reason of State and no matter what you decide in the end, the achievement will then unlock.

You must complete the Brothers in Arms quests.

There are a limited number of fistfights available.



Even Odds*[witcher.fandom.com]
Kill 2 monsters you have a contract on without using signs, potions, mutagens, oils or bombs.

IMPORTANT : The VERY MOMENT you accept a contract you must NOT use any signs, potions, mutagens, oils or bombs until you have completed the contract.

For example : If you decide to accept a contract, live it idle and then use a sign while doing something else, then even when killing the beast without using any signs, potions etc. it still won’t count towards the achievement.
Also make sure to completely remove the oil from your weapons : having 0/10 still counts as having oil.

During the quest Scenes From a Marriage you have to recreate all of Iris’ memories.

After the dining room memory and reading the letter. You will be facing 6 zombie-like Olgierds (ethereals). You will need to provoke all of them (hitting them with a sign or a sword) all while not killing a single one until all 6 are awake.


I Wore Ofieri Before It Was Cool*[witcher.fandom.com]
You must complete From Ofier's Distant Shores treasure quest to get all the Ofieri diagrams
Then buy an Ofieri decorated Saber, Ofieri saddlebags and Ofieri blinders from Dulla kh'Amanni, the merchant
Finally challenge Dulla kh'Amanni to a horse race (by mentioning his horse), you must win to get the Ofieri stock saddle
IMPORTANT : The saddle is unique, so do NOT sell the saddle before getting the achievement, you need to have all Ofieri equipment in your inventory to get the achievement



⭐Blood and Wine

David and Golyat[witcher.fandom.com]
As soon as you arrive in Toussaint, you will meet Golyat. To get the achievement you need to kill Golyat with a crossbow bolt to his eye. You do not need to get him low health since this shot will kill him instantly.
Using the Northern Wind bomb will help considerably.


A Knight to Remember*[witcher.fandom.com]
You must complete The Warble of a Smitten Knight BEFORE Capture the Castle.
You also must win all the 4 competitions thus you have to choose to remain in the tournament.


The Grapes of Wrath Stomped*[witcher.fandom.com]
First start Wine Wars: Belgaard. You must NOT SIDE with either one of the vineyard
Then you must do Wine Wars: Coronata and Wine Wars: Vermentino
Then Wine Wars: The Deus in the Machina will pop up, you must complete it.

You must complete There Can Be Only One secondary quest.
This can technically be missable as first you have to start the secondary quest There Can Be Only One, and there are a limited number of tasks that count towards each of the 5 virtues, though there's so many, you'll likely get one of each soon enough.

There are several missable gwent cards that you can only obtain at a certain point of the game

• During the side quest A Matter of Life and Death there'll be a small gwent tournament going on in the main courtyard where you'll need to win all the rounds to get all the cards.

To get this, you'll need to max out the Acquired Tolerance ability first, to boost your toxicity tolerance.

Perform 10 effective counterattacks in a row without getting hit or parrying.
This can easily be done during the tutorial section.
This is also achievable during fistfights.


Triple Threat[witcher.fandom.com]
Kill 3 separate opponents in one fight using 3 different methods (sword, bombs, crossbow, Signs, etc.)



Butcher of Blaviken[witcher.fandom.com]
Kill at least 5 opponents in under 10 seconds.


Can't Touch This![witcher.fandom.com]
Kill at least 5 opponents without taking damage and without using Quen sign. 

When surrounded by a pack of wolves, use Aard (Sweep) which will stun them and let you insta-kill them.
Don't hesitate to use bombs and Igni.
Early game you can mount Roach and hit several enemies without getting hit.

Kill 50 opponents using the environment.

Make good use of explosive barrels or swamp gas as shown in the video.
Use Aard on vulnerable monsters, like harpies and sirens, to get them to hit the ground and strike for an instant kill.

This can be achieved easily during the tutorial.

Make an opponent suffer from bleeding, poisoning and burning simultaneously. Do this 10 times.

Tracking bolts, despite saying they have a bleed effect, do NOT count for this achievement.

Kill 50 human and nonhuman opponents by striking them in the head with a crossbow bolt.

You will also know you got a successful headshot when the screen shakes a bit after.
(The rumour mentioned in the video about having to damage enemies exclusively with the crossbow is false.)
 
When using the 2 methods bellow, you do not need to manually aim, auto-aim will always give a headshot :

• When it's raining, use Superior Thunderbolt, it has a 100% critical hit chance during storms.
  
• Superior Samum gives a 100% Critical hit chance when it successfully stuns the opponent.

You can also use Northern Wind to manually aim better at the opponent's head.



Humpty Dumpty[witcher.fandom.com]
Kill 10 opponents by knocking them off somewhere high with Aard.
 
Aard Sweep can help as you don't need to aim at the ennemy to knock them off.
It's easier to do this on lower difficulties as the fly effect is substantially decreased at higher settings making it harder to send opponents flying.

Use Axii to force an opponent to kill another 20 times.
Make sure you have maxed out the Puppet ability for this and make sure to disable the Delusion ability, otherwise you won't be able to use Puppet.

Have all possible Place of Power bonuses active at the same time.
This is easily done in White Orchard. There are 6 total places (2 are Quen) with enough room to get them all before the first one expires.


Rad Steez, Bro![witcher.fandom.com]
Slide downhill uninterrupted for at least 10 seconds.
An easy spot is during The Path of Warriors. After reaching the top of Yngvar's Fang, the path automatically shows to go down a hill to the second part of the quest.
Sliding down this will easily net the achievement.

Kill 3 opponents with their own arrows.
 
 
You'll need to max out the Arrow Deflection ability first to try for this achievement.

Kill a frozen opponent with a crossbow bolt.
 
To get it, simply use Northern Wind to freeze the opponent then shoot at them.
Use only standard bolts. Ones like split bolts don't appear to work towards the achievement.

• Can you get all the achievements in New Game Plus?
  
  Yes and this also includes the Difficulty related achievements
  Note : If your first playthrough wasn't on Steam and you already had all the bomb diagrams then you won't be able to unlock the Bombardier achievement. Check wiki on how to get the achievement quickly
  
  
  
• For the Fast and Furious achievement, do you need to the win the optional race against Morvran Voorhis during the Broken Flowers quest?
  
  No, you don't need to win against him
  
  
  
• What specifies a contract being accepted with the achievement Even Odds - does it begin when I read the contract and the quest appears in my journal, or when I talk to the issuer and accept the contract?
  
  It begins the very moment you talk to the contract issuer and accept the contract.
  
  
  
• For the Even Odds achievement :
  Between the time I accepted the contract from the issuer and the time I completed the contract, I didn't use any signs, potions, mutagens, oils or bombs. Yet after doing this twice I still didn't get the achievement.
  
  Make sure to completely remove the oil from your weapons : having 0/10 still counts as having oil.
  To completely remove the oil, drop your swords and equip them again, the oil count should now be gone.
  (You can still have active abilities)
  
  
  
• Is it possible to 100% the game in one playthrough?
  
  Almost, the only achievements you can't get in one playthrough are Last Action Hero and Kling of the Clink from the Blood and Wine expansion.
  
  
  
• Will using console cheat codes void all Steam achievements?
  
  No it won't
