@cyborg sometimes when you start your left 4 dead 2 you'll need to wait a little bit so that all your addons can be loaded, try that and start or join a coop/vs/scavenge game.
if it doesn't work check you addons if you have another death music activated, if it is uncheck it.
if it still doesn't work.delete the file in the addons/workshop folder of left4dead2. then redownload it.
