I can tell you that in Galician-Portuguese the medieval name for mayor was Alcaide, and in Spanish it is Alcalde. Then, it could be Senhor Alcaide and Señor Alcalde for count republic, Alto Alcaide and Alto Alcalde for duke rank republic, Príncipe-Alcaide and Príncipe-Alcalde for king rank republic and Grão Príncipe and Gran Príncipe for emperor republic rank.
And the patricians can be called "burguês" in Galician-Portuguese and "burgués" in Spanish, it means "bourgeois".
Another thing I would change is the word barón/barão and baronia, because that tittles were not used in Castille, Portugal, Galicia and León. I think that it fixed better "senhor" (Portuguese) and "señor" (Spanish) or if you prefer, "castelão" and "castellano" (a person who rules a castle).
Could you change culture names for south slavs to be more accurate?
For Croatian its "hrvatski", for Serbian "srpski", Bosnian "bosanski", Carantanian "karantanski"... they end on "-ski" just like the polish version.
Also in those languages kingdom is "kraljevstvo".

Its not a mod. Titular titles for various German kingdoms in the Holy Roman Empire were added in one of the updates. So there's a titular kingdom of Franconia, and Swabia and Thuringia now.
