Mrs. Beakley will show up on all maps, btw. It takes a bit of searching but she's found in the most used paths. The only one I haven't spotted her on yet is The Moon and Mt. Vesuvius.

Just played through this game today, so I can provide an answer for that. Thankfully the game's interface shows you where to go on the map.

After collecting all three pieces and have entered the mirror, follow the map. The map will tell you where you need to go in order to face the final boss. Once you've entered the skull icon's proximity, the cutscene will play.

On Transylvania, after you get all three pieces and you go to the mirror, where do you go next? I Don't know if I am missing something really obvious, but I dont see how to get to the final boss room.

Mrs. Beakley only shows up in the Amazon, African Mines, and the Moon on Hard as well, not just Extreme.
