To anyone that's having issues, try the following:

1. Close L4D2
3. Unsubscribe
3. Delete "157498318.vpk" from the "addons\workshop" folder
4. Resubscribe
5. Open L4D2 and let the addons download/sync

Is this mod only for local servers?

I meant let steam finish downloading and syncing the workshop addons after resubscribing. It can take a few seconds before they're installed. You can see when it finishes in the "downloads" section in your steam library page.
