You will receive this investigation from Usher Talltree, who is in the basement behind the Temple Church (Nothern part of the WEST-END).

The earliest time to go there is in Chapter 5 when you are going to reach the Dawson's mansion.
(Actually you can find him in Chapter 4. Thanks aubri for informing me that.)

However, it will not allow you to enter the THEATER until you follow the main story to rescue Swansea at the end of Chapter 5.

You will find a lcoked safe in the basement of the theater and there is also a pinned note on the desk closed to the safe.

Pick it up and you will trigger the next step: go to the right of the second floor(you can use your sensory ability)to kill two guys and you will get the key.

Return to the safe and open it you will get the Usher Talltree's notebook.

REMEMBER: DO NOT PEEP THE NOTEBOOK OR YOU WILL GET NOTHING FROM HIM.

Then bring back the notebook to Usher and you will get a collectible named ORIGIN OF THE BROTHERHOOD. There is a pattern in the upper right corner of this paper. Here is what mine looks like:

It's said that different people have different positions of the four symbols above the pattern.

The puzzle of the four slates

You must notice that there are four active slates downstairs at the entrance of the basement.

The position of the slates correspond to the position of the four symbols in the pattern.

If you are careful enough, you will find out there are some strange symbols with dots around them in some collectibles of which the authors are all the same person Usher Talltree.

Open sesame

You may now realize that the number of dots around the symbols is the order you step on the slates.

Do it !

REMEMBER YOU MUST WAIT FOR A SLTAE TO DROP COMPLETELY AND THEN STEP ON ANOTHER SLATE.

After you finish 6 steps, the stone gate will open for you.

There are a note named RECOLLECTIONS OF PAULUS AURELIANUS and a sword on the stone platform.

So there you will unlock the achievement.

As for the locations of the 6 collectibles, I'm sure that it is easy for you to check on some walkthrough websites and I make few statements here.

I missed the "Laughing at the guards", in the guard of Priwen section, when I was going to try this puzzle, and it didn't make much sense. Or, well it sorta did, but I felt something was missing. It might be one of the last notes you get. At least I was faced with an ichor werewolf in Southwark foundry, and he dropped a key to the 3rd floor of the building. It had one of the symbols to help out for the puzzle.

That's not right, I had Fish, Circle, Square, Circle, Fish, & Diamond, in that order. That would make it 1, 2, 4, 2, 1, 3.
I was trying over and over with the solution of the two comments below me, to no avail. But I tried this order once and boom!

So the order of the plates is always 1,2,3,2,1,4...

They are always cross for me but that doesn't mean they will be for you... and yes you can unlock it without the paper clue and get the achievement
