The model was made by Richard P as part of his modders low poly car pack 2 for Need for Speed 4 available here: NfS4 Version [www.nfscars.net]
I added the wheels, edited mesh and textures, added additional texture maps and converted it to Cities: Skylines.

Technically, it should be a Colony Park; the Country Squire was the Ford version.
