Die Osteoporose (von altgriechisch ὀστούν ostoun ‚Knochen‘ und πόρος poros ‚Furt, Pore‘) ist eine häufige Alterserkrankung des Knochens, die ihn anfällig für Brüche (Frakturen) macht. Die auch als Knochenschwund bezeichnete Krankheit ist gekennzeichnet durch eine Abnahme der Knochendichte infolge eines den Aufbau übersteigenden Abbaus von Knochengewebe im Rahmen des natürlichen Knochenumbaus. Aufgrund der höheren Umsatzrate ist die Spongiosa typischerweise stärker betroffen als die Kortikalis, was sich in den Prädilektionsstellen für Knochenbrüche niederschlägt; die erhöhte Frakturanfälligkeit kann allerdings das ganze Skelett betreffen.
Die Osteoporose, welche erstmals 1885 von dem Innsbrucker Pathologen Gustav Adolf Pommer (1851–1935) beschrieben wurde, ist die häufigste Knochenerkrankung im höheren Lebensalter.
