Cloak & Dagger - Spy watch, regens cloak when not moving, can't refill cloak from ammo drops
Mantreads - Soldier, replaces shotgun I think. Only take 25% of self rocket damage when rocket jumping, deals 3x falling damage to player you land on
Chargin targe - Demo shield , replaces a grenade launcher, forget which one, 50% fire resistance on wearer, 40% explosive resistance on wearer, right click makes you charge and gives attack power bonus (but can't control charge unless you have another item)
Razorback - Sniper, replaces SMG, stops 1 backstab from a spy and plays a loud electric sound when that happens, giving you time to flee.

Usually playing what's needed... as for items... I have these spare and am willing to trade:

Supply Crates x4
Sandman - Scout bat, fires a baseball that stuns opponents, -15 health on wearer
Atomizer - Scout bat, grants 3rd jump, 30% slower swinging speed, 20% less damage, take 10 damage on 3rd jump
Bushwacka - Sniper machete, crits when it would normall mini crit, take 20% more fire damage
Huntsman - Sniper bow.
Backburner - Pyro flamethrower, 10% more damage, 100% of crits when attacking from behind, +150% airblast cost
Kritzrkrieg - Medic healgun, gives 100% crit rate instead of invuln
Ambassador x2 - Spy gun, crits on headshot, -15% dmg penalty, 20% slower firing speed
Gunslinger - Engi wrench, gives quick-building 100 metal mini sentry instead of normal sentry
Dead ringer - Spy watch, makes you 'feign death' when active and cloaks you. Can't control cloak otherwise.
Half-zatoichi - Soldier/Demo sword, restores you to 100%hp on kill, but can't be sheathed until you kill someone

Looks like we're all playing a hefty amount of TF2. (213 hours total at time of post!)
