How do you unlock the LOSE ALL TREASURES and AVOID EVIL FACE FOR ONE MINUTE achievements?

When you clear a area of all enemies and leave through a door to another area and then return to the area that was previously cleared, are all enemies still all cleared or do they respawn? Is there a overall map of all the areas?
