How to unlock all skills?

I've done 2 playthroughs now trying to get this all skills achievement but it seems impossible? I'm always missing 2 points for the last unlock. Is there a trick I'm missing or whats up?

Mastery Achievement Broken WHY ?

I beat this game the 2nd time without EVER being defeated and I STILL don't have the mastery achievement and the developer of this game said all you have to do is beat the game without being defeated.
