When using 3ds max, I just made sure that the model is properly aligned with the axes, and ideally located in the center of the x and y plane (0,0). The rotation and scale can be adjusted in-game once you've exported your model, or you could adjust them in 3ds max via the heirarchy tab (fixing the pivot) and setting your unit scale to 1 unit = 0.001 meters.
