I believe it is this mod https://steamproxy.net/sharedfiles/filedetails/?id=235706478&searchtext=hud for the hud itself and https://www.gamemaps.com/details/6161 is for the animated health bars

No sorry i had to brighten the photo, (its not a great photo im going to change it when i get around to it lol)

however you can get your game to look like that with sweetfx/enb, but i dont bother using it anymore though since i use the color correction mods which darkens the game a little bit and changes the atmosphere
