UPDATED: 9 February, 2022
Story Achievements - Part 1
The following achievements, as part of Shadows of Amn, are story-related and thus unmissable. However, some will require doing specific actions to unlock:

A Daring Escape
Reach Waukeen's Promenade during Chapter 1.

Picking Sides
Complete Chapter 2.

Across the Sea
Complete Chapter 3.

Friend of Darkness / Shadow Ally
Both achievements focus on the same condition - side with a faction to complete chapter 2 upon reaching 15,000gp, then complete their quests before departing for Brynnlaw to complete Chapter 3. There is a catch though.

The easiest of the two is Shadow Ally, as this achievement is hard to miss since you can ally with the Shadows Thieves at any time. Friend of Darkness is different, as you must ally with Bodhi when asked to meet her upon reaching 15,000gp - you can't stall joining up with her, as you only are given the choice there ans then to join her.

However, it is possible to get both achievements in a single playthrough by saving before reaching 15,000gp, as the meeting reqest with Bodhi is triggered upon reaching this amount. Since Friend of Darkness is missable because of condition mentioned above, I recommend doing this first, even if you are not intending to ally with her during the playthrough.

Time to Slay
Become the Slayer for the first time. Although there is a scripted scene in Chapter 4 when that happens, the achievement will unlock when your main character randomly becomes the Slyer after that point.

Unknown Risks / Took a Chance
During Chapter 4, choose one of the following towards completing the Spellhold dungeon:
  • Unknown Risks - Use the portal on the upper levels of the Asylum, after finding the key to unlock the door leading to it. This completes Chapter 4.
  • Took a Chance - Escape from the Asylum, and return to Brynnlaw. Then seek out Saemon Havarian and complete his help to secure another ship. This leads you to dealing with a new area before you can complete Chapter 4.
Both achievements can be acquired in a single playthrough by saving before completing one path, reloading, and then playing out the other path.

Inmates Running the Asylum
Complete Chapter 4.

Return to the Surface
Complete Chapter 5.

Staked
Complete Chapter 6.

To Hell and Back
Complete Chapter 7.
Story Achievements - Part 2
The following achievements, as part of Throne of Bhaal, are story-related and thus unmissable. However, some of these may require completing certain actions to unlock:

Hell's Pocket
Defeat Illasera in the Grove of Ancients during Chapter 8.

The First Challenge
Complete the First Challenge within the Pocket Plane. This must be completed in order to progress in Chapter 8.

Felling the Giant
Defeat Yaga-Shura during Chapter 8.

The Second Challenge
Complete the Second Challenge within the Pocket Plane. This becomes available after defeating Yaga-Shura.

Dragon's Demise
Defeat Abazigal during Chapter 9.

The Seven Statues
Defeat Sendai during Chapter 9.

The Third Challenge / The Fourth Challenge
Complete the Third and Fourth Challenge within the Pocket Plane. The third challenge becomes available after defeating either Abazigal or Sendai first, and the fourth when the other Bhaalspawn is defeated.

Solar Supremacy
Defeat Balthazar during Chapter 9.

The Fifth Challenge
Complete the Fifth Challenge withint the Pocket Plane. The challenge becomes available after defeating Balthazar.

Sieze the Throne / Heavy Is the Head
Complete the final fight then chose which of the following in the conversation with the Solar to end the game:
  • Sieze the Throne - Simply accept the Solar's offer.
  • Heavy Is the Head - Simply refuse the Solar's offer.
Both achievements can be acquired in a single playthrough by saving before the final fight, completing one path, reloading, and then playing out the other path.
Side Quest Achievements
The following achievements are unlocked by completing specific side quests in Shadows of Amn, upon reaching Chapter 2. While these must be earned before completing Chapter 7, they can only be completed during Chapters 2, 3, 6 & 7:

The Great Detective
Complete the quest "Solve the Skinner Murders in the Bridge District" - The quest automatically begins when entering the Bridge District for the first time.

Best Friends
Complete the quest "Put the Spirit of the Child Wellyn to Rest" - The quest is located in the Graveyard District, but can only begin and end when you visit at night.

New Ownership
Complete the quest "Free Hendak and the Slaves" - The quest is located within the Copper Coronet, and requires gaining access via Lehtinan.

Heart of the Abyss
Complete the quest "Inside the Planar Sphere" - The quest is located in the Slums District, but requires you to first complete the quest "The Hunt for Valygar Corthala".

Close the Eye
Complete the quest "Investigate and Destroy the Cult of the Eyeless" - The quest is located in the Temple District and begins automatically after witnessing a scripted scene outside the Temple of Lathander.

Fang and Claw / Trade Must Flow
Complete the following quests:
  • Fang and Claw - "Animal Trouble in Trademeet"
  • Trade Must Flow - "Dealing with the Trademeet Gennies"
Both quests are located in Trademeet, which is marked on your world map after conversing with Flydian at Athkatla's City Gates.

Light in Darkness
Complete the quest "Investigate the Deaths in the Umar Hills" - The quest will unlock if the NPC Delon is conversed with, either by finding them in the Government District, or approaching the party with Minsc as a companion for a set period of time.

No Child Left Behind
Complete the quest "Journey to the Windspear Hills", to trigger a scripted scene. After this has been completed, you begin the quest "Rescue Gerran's Child from Firkraag". Venture to the dungeon in Windspear Hils and locate Iltha within. Release her to unlock the achievement; you do not need to complete the quest or kill Firkraag to do so.

To the Limit
Complete the quest "Dennis' Mother's Gong". To unlock the quest, acquire and cast the spell "Limited Wish". When the Djinn from the spell appears, ask for "an adventure like none you've ever had before" amongst the choices given, and then read the note he leaves you.

A Battle for the Ages
Complete the quest "Restore Kangaxx's Body" - The quest is located in the Docks District, in the building between the Sea Bounty and Mae'Var Guildhall in the Docks District.
Companions Quest Achievements - Part 1
The following achievements are unlocked by completing companion quests in Shadows of Amn. These become available from the outset of Chapter 2 and must be unlocked before completing Chapter 7. Like the Side Quest achivements, they can only be unlocked when the quests are completed during Chapters 2, 3, 6 & 7:

I Dub Thee
This achievement is more extensive than the others, because it involves additional actions upon recruiting Anomen:
  • Keep the party's reputation level above 16.
  • Complete the quest "Anomen Returns Home After his Sister's Death", by having Anomen seek justice not revenge.

Bring Anomen to the Order of the Radiant Heart - roughly around 10 in-game days after his quest is completed - and the achievement will unlock if both conditions above are met.

Reunited
Recruit Cernd in Trademeet and return with him to any district in Athkatla (except the City Gates) to begin the quest "Cernd's Child". To unlock the achievement, complete the quest by choosing the option to support Cernd in threatening Deril, so that he is reunited with his child. Note the quest is time sensitive, and the other options may not unlock the achievement.

The Nether Scroll
Complete the quest "Find the Nether Scroll for Edwin" - Begin the quest "Find Proof of Mae'Var's Treachery" in order to find and recruit him, then take him to the Graveyard District to active his quest.

Planehopper
Complete the quest "Free Haer'Dalis from Inprisonment by Mekrath", followed by the quest "Rescue Raelis and Haer'Dalis from the Planar Prison".

Restored
Complete the quest "Save Jaheira from Baron Ployer's Curse" - This quest activates when Jaheria is in the party when visiting the Sea Bounty in the Docks District.

Homecoming
Complete the quest "Jan Jansen Returns Home" - Recruit Jan in the Government District after siding with him during a scripted encounter. After 15-18 in-game days, a scripted scene will occur, and the quest begins automatically.
Companions Quest Achievements - Part 2
Family Matters
Complete the quest "Keldorn Wishes to Visit His Wife and Family" - Recruit Keldorn and have him in the party when visiting the Government District to activate the quest.

The Book of Kaza
Complete the quest "Help Korgan Recover the Book of Kaza" - Korgan must be allowed to be part of the party in order to begin and complete the quest.

Sisters
Complete the quest "Save Mazzy's Sister, Pala" - Recruit Mazzy to the party either during or after completing the quest "Investigate the deaths in the Umar Hills". After some in-game time with the party, her quest will unlock following a scripted scene. Make certain to complete the first task in the quest - Mazzy will leave if you fail to do so within a reasonable amount of time, making the achievement missable.

Absolute Power
Complete the quest "Nalia Abducted by Isaea Roenall" - Complete the quest "The de'Arnise Keep has been Invaded". Recruit Nalia to the party either during or after it. At some point she will request you visit the Graveyard District. After the scripted scene upon arriving, the quest will begin automatically a few in-game days later following a scripted scene that sees Nalia missing from the party.
Stronghold Achievements
The following achievements are unlocked by acquiring the main character's associated stronghold per their class. Each class has a specific stronghold that is unlocked by completing its respective side quest, and the player is usually directed towards that quest during the final conversation in Chapter 1.

A dual/multi-classed character can acquire one of the available strongholds to them, but will lock out the others as a result (i.e. a Fighter/Thief can chose to claim the Thief stronghold, but will not be able to claim the Warrior Stronghold as a result). A player can use dual/multi-classed characters to their advantage by saving before taking one stronghold, and then reloading after acquiring its achievement to secure another. Even so, you need to do multiple playthroughs to acquire all of these:

I'll Keep This
Complete the quest "The de'Arnise Keep has been Invaded" with a Main Character whose class is either Fighter, Monk, or Barbarian. The stronghold is unlocked by accepting it from Nalia at the end of the quest.

Spherical Stronghold
Complete the quest "Inside the Planar Sphere" with a Main Character whose class is Mage or Sorcerer. The stronghold is unlocked by accepting it from Lavok at the end of the quest.

Guild Times
Complete the quest "Find Proof of Mae'Var's Treachery" with a Main Character whose class is Thief. The stronghold is unlocked by accepting it from Renal at the end of the quest.

Divine Domain
Complete the quest "Investigate and Destroy the Cult of the Eyeless" with a Main Character whose class is Cleric. The stronghold is unlocked by accepting a place at the end of the quest, from the temple the player took the quest from.

Orderly Surroundings
Complete the quest "Rescue Gerran's Child from Firkraag" with a Main Character whose class is Paladin. The stronghold is unlocked by visiting the Order of the Radiant Heart in the Temple District after the quest, and accepting the offer from Prelate Wesslyn.

Cabin in the Woods
Complete the quest "Investigate the Deaths in the Umar Hills" with a Main Character whose class is Ranger. The stronghold is unlocked by accepting it from Minister Lloyd at the end of the quest.

Home Grove
Complete the quest "Animal Trouble in Trademeet" with a Main Character whose class is Druid. The stronghold is unlocked after defeating Faldorn and accepting to take over as leader of his grove.

The Play's the Thing
Complete the quest "Rescue Raelis and Haer'Dalis from the Planar Prison" with a Main Character whose class is Bard. The stronghold is unlocked after receiving the deed to the playhouse in the Five Flagons at the end of the quest.
Difficulty and Endgame Achievements
The following achievement are connected to completing the game under certain conditions. It is not possible to achieve all of these in one playthrough, thus you will need to get them in at least two playthroughs to achieve 100%:

Flying Solo
Complete the game with Main Character only. You must not recruit any of the game's companions during your playthrough.

Friend of Volo
Complete the game on Story Mode, without changing the difficulty.

Lord of Murder Achievement
Complete the game on Legacy of Bhaal, without changing the difficulty.

Leave No Friend Behind
Complete the game without no companions in the party dying during your playthrough.

Sacrifices Must Be Made
Complete the final battle in Chapter 10 with only the main character alive - any and all companions must be dead before it is over. The achievement is voided if at least one is still standing. The achievement is not possible if on Story Mode.
Character Statistic Achievements
The following achievements are unlocked by attaining certain statistic with either the main character - either created for Shadows of Amn, Throne of Bhaal - or companions. These can also be achieved by characters created for The Black Pits 2: Gladiators of Thay:

Epic Warrior / Epic Rogue / Epic Priest / Epic Mage
Each of these achievements requires a created character to reach level 30 in a specific class, regardless of using a kit for that class. This is much easier with Gladiators of Thay:
  • Epic Warrior - Character must be Fighter, Paladin, Ranger, Monk or Barbarian.
  • Epic Rogue - Character must be Thief or Bard.
  • Epic Priest - Character must be Cleric, Druid or Shaman.
  • Epic Mage - Character must be Mage or Sorcerer.

Godlike
Increase a character's attirbute score to 25. This is easily done by using multiple Potion of Mind Focusing, since the effects of that potion are stacked.

Juggernaut
Increase a character's health to 150HP. The easiest way to do this is start a new game on Throne of Bhaal, and creating a half-orc fighter with high Constitution at around 19 - doing so will leave them with around 167HP when the game begins.
Author's Note: Thanks to Yamian for this info.

Untouchable
Increase a character's armour class (AC) to -15 or less. The easiest way to do this is to create a Thief character with the Blade class kit, and set them up with 18 Dextrity, which will give them -4 AC. During the main game, acquire and equip them with Spirit's Shield+2, cast the Lv4 spell Spirit Armour, and perform the Blade's Defensive Spin action. All of this should reach the required armour class.
Author's Note: Thanks to Yamian for this info.

Loremaster
Increase a character's Lore Skill to 100. This achievement is easily done by either importing a Bard character from Baldur's Gate: Enhanced Edition that achieved that game's "Loremaster" achievement, or create a Bard character for Throne of Bhaal.

Grandmaster
Have a character be a Grandmaster with any weapon. This is best achieved by creating a character whose class is Fighter, as they can reach Grandmaster level on a weapon once they are around Level 8-9. This achievement is much easier to get compared to its counterpart on Baldur's Gate: Enhanced Edition.

Hero / Villain
Have your main character complete one of the following:
  • Hero - Simply be good and complete quests of a good nature to improve your reputation level to the maximum level of 20.
  • Villain - Simply begin killing peasants and neutral characters to lower your reputation level to the lowest level of 1.
Both achievements can be acquired in a single playthrough by saving before completing one path, reloading, and then playing out the other path.

Oathbreaker
Have your main character reach a reputation level of 1 as either a Paladin or Ranger. See Villian on how to achieve this.

Both achievements can be unlocked in a single playthrough by saving and choosing one path, and then reloading after tat and doing the other path.

Murder Incarnate
Kill 1,000 creatures during a playthrough. Since kills in ambushes are counted towards this, the best thing to do is travel around the world map in Shadows of Amn, and trigger ambushes in doing so.
Miscellanous Achievements - Part 1
The following achievements are miscellaneous tasks that don't have any quests associated to them. These must be done in specific chapters of Shadows of Amn:

Wand Collector
Disarm all six wand traps on Level 2 of Irenicus' Prison dungeon, during Chapter 1. These must be disarmed before leaving the dungeon, and can only be deactivated by finding each trap's corresponding wand key. There are six keys to find:
  • Dropped by the Otyugh on Level 1.
  • In a bedroom on Level 1, beyond the location of the Otyugh.
  • In the room where Khalid's corpse is found on Level 2; there are three keys in here.
  • Dropped by the Clone of Ellesime on Level 2, in the room near to where Khalid's corpse resides in.

Friends Like These / Peace At Last
Both of these achievements require that you have Yoshimo in your party when you are ready to advance to Chapter 4. Friends Like These will unlock when you kill him while in Spellhold. The other, Peace At Last, will require you to loot Yoshimo for Ysohimo's heart after the fight with him. Once you reach Chapter 6, bring the item to the Temple of Ilmater in either the Slums District or Waukeen's Promenade. Leaving Yoshimo behind at the end of Chapter 3 will void acquiring these achievements.

Freedom
During Chapter 5, use the spell "Freedom" on Vithal, north of the Duergar camp. If you do not have the spell, you can purchase a scroll with the spell from the camp.

Double Cross / Triple Cross
For the main quest "Rescue Adalon's Eggs" in Chapter 5, do not attempt to steal the eggs while your party is disguised as drows. Instead, work with Solaufein and complete his tasks. After you are sent to work with Phaere, complete her tasks, until she asks you to kill him. Your decision will affect which achievement you will unlock, at which point do one of the following:
  • Double Cross - If you kill Solaufein, then when you continue the quest, Phaere will eventually request that you switch the dragon eggs within the temple in Ust Natha with fakes. Agree to this, complete the task and bring the real eggs back to her. The achievement unlocks after accompanying her to the temple.
  • Triple Cross - If you spare Solaufein's life, the situation will remain the same as above, but with a big difference. After you agree to Phaere's request, Solaufein will request you give her fakes as well. Agree to this, and complete Phaere's task as before, but when you return to her, give her Solaufein's fakes. The achievement unlocks after accompanying her to the temple.
Both achievements can be acquired in a single playthrough by saving before completing one path, reloading, and then playing out the other path.

The following achievements are connected to miscellanous tasks that have no quest to them, and can be done at any time between Chapter 2-3, or Chapter 5-7:

Twisted Victory
Enter a locked door in the Bridge District, and defeat the creatures within. The door is near to Delosar's Inn, and requires a Rogue Stone in the party's inventory to open it. Be prepared for the fight beyond it.

Don't Mind Us
Kill the mind flayers within their lair in the sewers of the Temple District. The lair's entrance requires a key, which is dropped by Tazok when you kill him in the dungeon in Windspear Hills. Be prepared for the mind flayers before fighting them.

Fury of the Heavens
Find and indentify the weapon Celestial Fury +3. The weapon can be found within the Guarded Compound in the Temple District, after provoking a fight within.

The Holy Sword
FInd and identify the weapon Carsomyr +5. Part of the loot dropped by Firkraag in the dungeon in Windspear Hills.

Legendary Relic
Acquire the weapon Crom Faeyr +5. To do this, you must bring to Cromwell, in the Dock District, the following items to receive the weapon:
  • Scroll of Crom Faeyr - Dropped by Thaxll'ssillyia in the dungeon in Temple Ruins.
  • Hammer of Thunderbolts +3 - Found in tthe sewers in the Temple District with the mind flayers.
  • Girdle of Frost Giant Strength - Dropped by a Demon Knight in the Kuo-Toa Dungeon.
  • Guantlets of Ogre Power - Dropped by a halfling warrior in the Planar Sphere
  • 10,000gp

Ancient Power
Acquire the Robe of Vecna. The item is for sale in the Adventurer's Mart in Waukeen's Promenade.

In Balduran's Image
Equip your main character with the equipment of Balduran (class permitting). Their are four items that have in their name "...of Balduran" - Helm, Sword, Shield, and Plate Armour. While the Helm must be recovered from Irenicus' Prison before you leave it, the others can be purchased at the Adventurer's Mart in Waukeen's Promenade.
Miscellanous Achievements - Part 2
The following achievements involve killing four dragons that are/can be encountered in Shadows of Amn. These enemies are tough to fight, but can be dealt with using a good dose of spells, such as Bless and Chant, that can protect from negative effects, and improve armour class, saving throws and THAC0. These fights can be made easier by using the weapon weapon Carsomyr +5, except with Firkraag, as he drops it when killed:

Red Dragon Slayer
Kill the red dragon Firkraag in the dungeon in the Windspear Hills.

Shadow Dragon Slayer
Kill the shadow dragon Thaxll'ssillyia in the dungeon in the Temple Ruins.

Silver Dragon Slayer
Kill the silver dragon Adalon in the Underdark. The dragon will not be hostile when encounted during Chapter 5, but will if you accept her quest and then return from Ust Natha without her eggs.

Black Dragon Slayer
Kill the black dragon Nizidramanii'yt in Suldanessellar. This dragon is encountered during Chapter 7.

The following achievements are for miscellaneous tasks that have no quest, but are only accessible during Throne of Bhaal:

Welcome, Brother
During you first visit to the Pocket Plane, you will encounter Sarevok. Agree to resurrect him, then add him to the party.

A Sister's Sacrifice
Enter Chapter 8 with Imeon in the party. Upon entering the Pocket Plane and encountering Sarevok, convince Imeon to resurrect him.

Green Dragon Saviour
Find the green dragon Fil'Yissetat in Abazigal's Lair during Chapter 9, and use a scroll of "Reversal" on them. A scroll with that spell can be found within the location.

The following achievements concern miscellanous tasks that can be achieved at any time:

Best Day Ever
Have a character under the effects of Bless, Luck and Chant at the same time. Both Bless and Chant are AOE priest spells, while Luck is a mage spell that must cast directly on the target character.

Worst Day Ever
Have a character under the effects of Fatigue, Poison and Disease at the same time. Fatigue can be achieved from either having a character not rest for long hours or from using the spell "Haste", which triggers the debuff when the spell's effect wears off. The mage spell "Contagion" confers Disease, and the priest spell "Poison" can confer the respective effect, though quicksave when using these spells in case a saving throw blocks them. Don't attempt this in Story Mode, and note that Poison can interrupt spellcasting if self-inflicting the conditions to the character.

Not quite related but something I noticed; Cavaliers cannot use *most* ranged weapons, but for some reason they can use one. They can throw the Hangard's Axe +2 (Returning Axe).
