if you're right, then they have changed it for the best;
in early versions of the game player was obliged to finish a level with chosen weapon having at least one bullet in the chamber - I know that for sure because in my first attempt I finished the level without bullets and didn't get an achievement;
therefore, in this manual I decided to specifically bring up the fact that you need to have at least one bullet remaining - in this case you getting an achivement is guaranteed

I think I've found an easier way to get the "Undead Head Harvester" achievement.
When you first enter the sewers fighting against the rats....
I went into the sewers and was shooting each one with the tommy gun. Went to the end, jumped up the pipes and hit the switch and went back along the way I'd come. Before I'd even got to the new area I unlocked the achievement - just by tommy gunning rats. I wasn't even trying for it!
