To access the first glitch world, first make your way to the Pinwheel Funhouse. If you do not know how to get there, you get inside by walking into the purple tent furthest north in the Pinwheel Circus (Past Brow Boy).

Once you're inside, Walk into the purple tent to the right (Not up).



Next, walk around the gravestones until you are about 7 blocks in. Then, you should see a gravestone appearing to be enclosed in a stone wall to the south. Walk straight into this wall, and you will be teleported through the gravestone.



Once you are in this part of the funhouse, walk straight down and to the right, through the two tree stumps (Also fake). You will now have to defeat the secret boss, Bubba, in order to pass. Once you do, walk through the purple tent behind him.



Once through, keep walking east until you reach the wall, go one block down, and walk through that wall. You are now in another part of Blacktomb yard. Here is where the glitching begins.

Walk down and through the flashing the flashing stone, and you will be in glitch world level one. From here, walk down and southeast until you reach a pair of trees. From here, walk up (North) and enter the flashing tree.





Now, in glitch world level 2, walk 2 blocks north and you will see two objects to your right, one of which is flashing. Walk through the first object into the flashing one, and you will be in glitch world level 3.



Now, follow the path until you reach the intersection, where you will take a right. From here, go up and right again until you reach the wall on the right.



After that, go straight down from there walking through the fake objects below you, and follow this path until you reach the final flashing object.



From here, you can either talk to the fishing guy (Old Man Consequences) and get the "4th Glitch" trophy, or you can go even deeper.

Now here is how you reach the 5th glitch. Walk to the top of the pond, get as close as you can, and spam click the "A" and "D" buttons. You will gradually wade into the pond until you are completely under.



Once you reach that point, you will see your Freddy character quickly falling through darkness, presumably the 5th and final glitch.



Now, if you wait long enough, eventually the scene will switch and you will see what some believe to be Scott and his two boys, watching a color-changing screen.



At this point, you can't even press the ESC key, so when you've finished you will want to press your "Windows" key and close out the game. NOTE: You can also press the F2 key to restart the game, as pointed out by ✿《Little_ℒizbeth》✿.
