Maybe there is a quest related to all those bugs in the same area! xD
1) Geralt started the falling animation (I'm not english but I think you can understand xD) without stopping
2) Geralt was walking underwater
When i come back to desktop to start OBS the bug disappared :/
it is possible that this happens when you "overload" the game, because witcher 3 has a limited amount of textures accessibility. If this is the case try this :
