Dread Delusion is a great game but unfortunately it is littered with bugs, some of which can permanently ruin your playthrough since the game keeps a single autosave per character.
Backup your savefile manually every once in a while as a preventive measure.

3 achievements (Welcome To The Underlands, Cartographer, Master Cartographer) were bugged at launch but should now get unlocked retroactively if you meet the requirements.

• Game modes: one achievement requires reaching a certain point of the game (not the ending!) without dying before, and another one requires beating the game in Hard Mode.
  
• Story: two sets of mutually exclusive achievements.

Iron Will
Missable!


Can be obtained in Normal Mode. This achievement requires reaching a certain part of the game (NOT the ending!) without ever dying before, specifically a mandatory encounter with the Unnamed God after meeting Vela in the Underlands.

This is easier to achieve than it might sound, as enemies in the game barely deal any damage. If you happen to die it definitely won't be in combat. Keep Return Home spell equipped to be able to save yourself from fatal falls (or just exit to the main menu), make sure not to stand directly below any descending elevators, and maybe also occasionally backup your savefile - in case floor collision decides to take a break from its job. It does that from time to time.


Hardened Confessor
Missable!


Complete the game in Hard Mode. List of changes from the developers' patch notes:

• Increased enemy health by 50%
  
• Increased enemy damage by 50%
  
• Enemies respawn much faster
  
• Stamina regenerates 20% slower.
  
• Mana cost increased by 10%
  
• Fatigue build-up increased by 25%
  
• Potions must be bought or crafted. They no longer drop from enemies.
  
• Enemy rare drop rates halved.
  
• Herbs take an additional day to regrow.
  
• Enemy placement has been changed.

These achievements get unlocked automatically and can be ignored.

To The Cradle

There are two sets of mutually exclusive endgame story achievements.
If you want to get all four in one run then complete sidequest "The Whispers of Ghosts". Don't pass the Charm skill check in a conversation with Vela in the Underlands (or you'll get permanently locked out of an achievement), then backup your savefile before the final encounter with her. Pass the check and pick an ending -> restore the savefile -> don't pass the check and pick the other ending.


There Is Another Way
Missable! This achievement and A Legendary Clash are mutually exclusive

1. Complete sidequest "The Whispers of Ghosts".
   
2. Speak to Vela about her sister either in the Underlands or later in the Cradle and pass a 75 Charm skillcheck.
   
3. The achievement will unlock in the last encounter.
