The way to unlock the Innovations is a event called a "Critical Success" (event 9200). What determines if a Innovation is a failure, a success, or a C.S. is a random roll of
(65-SuccessChance) for Failure,
34 + (SuccessChance*0,75) for Success
and 1 for Critical Success.
So, the chances for a Innovation unlock on a 65% Success Chance research would be 1/83, which would justify why you haven't seen it yet.
The thing you said, about exhausting every option, doesn't apply, as the choice is made before you choose what reward you get (in the case of you exhausting every option, prestige)
