alright so here is what to do.

Step one: grab the tomahawk. PAUSE! WE ARE NOT GONNA HIT HIM IN THE FACE! Instead violently hit his rib cage which luckily does not cause blood loss!

Step two: grab his lung. Yes there is a glitch in the game that causes you to hit his lung losing blood but without blood loss at all!

When you get 1,000,000 dollars (as his fee), Donald's gonna be extremely close to death but this is a video game. NO NEED TO WORRY!

Lastly play the surgery like you would normally but make sure it's very careful one scalpel swipe will hurt the patient and cause death to occur.

Once you place said heart down. you will be able to complete the surgery with the achievement unlocked and if you saved it for last. You will be able to get the "There is nothing more to teach" achievement which you can get a blue ribbon with.
