If you want to learn how to extract addons, it is one line in the .lua that defines this weapon. Changing it is easy, extracting and recompiling the addon is a bit more complicated.

just convert the vtf into a png. After that, upload the png to your server at "vgui/ttt" make sure it's named "icon_medkit.png" and change line 16 in the weapon file to SWEP.Icon = "vgui/ttt/icon_medkit.png"
