it'd be a lot of work and I have done it myself for my personal server but it is only patched for the mods we use.
I definitely agree, I really don't think an item by item basis is possible (hopefully in the future)
I also know that... it's most likely not possible to do it on a item by item basis, since this mod relies on ItemTweaker, which has already a couple limitations, so I'm really sure it's not possible to do it like that, which means that, yeah, no open coats + bulletproof vests. Also, I forgor that bodylocations deals with exclusivity, so yeah, it's a base game problem.
group:setExclusive("Boilersuit", "TorsoExtra") would be the line doing that, it ONLY touches those 2 items conflicting and there are multiple more lines for other conflicting items, removing that line is all that you have to do to remove the exclusion, it won't touch things like TorsoExtraVest by doing that, unless you remove the line for TorsoExtraVest as well

@DrBossWatson I do have that same question, it shouldn't conflict, but it happens with base vanilla clothing, so it may be the bodylocation lua file, hopefully
