Step one: Play second level of The Black Labyrinth DLC (Mystic Shrine).

Step two: Find a waterfall and go through it.

Step three: On the back side of the cave there is a red target, shoot it.

Step four: Go through opened portal.

Step five: Defeat the creature.

The waterfall mentioned here is in the big area after the gold key door, for anyone looking.
