This guide will cover all 45 achievements from The Last Hero of Nostalgaia including DLC.

First, anything marked with an asterisk (*) has MISSABLE content.
I highly recommend that you read those sections first and be prepared for SPOILERS.

Second, you can't get all achievements in one playthrough. You need at least TWO playthroughs.
Some achievements are mutually exclusive (Lore of the Land vs Laid Bare, Innocent gift choices).
I'm grouping these achievements in Gameplay, Collectibles, Questlines/Story, and Miscellaneous.

Third, for some sections I will straight up link you to other guides that have been made.
Either guides that I have made or others who have put in their time and effort to make them.
So apologies in advance for making you flip back-and-forth between guides.

Anyway, let the guide begin and do not hesitate to correct me.
GAMEPLAY ACHIEVEMENTS
These achievements involve playing through the game's mechanics such as leveling up or using combat mechanisms.
1. Achievement
You just have to get an achievement. Yes, it's quite as easy as dying in a soulslike game.
2. It's a Soulslike
Speaking of dying, all you have to do is die to get this achievement.
3. Such Salt
Yes another achievement that requires you to die again.
You need to grind for a lot of souls memories that'll give you at least five levels then die.
Try not to lose these memories for the next achievement.
4. Memory Challenge
For this achievement, you need to have enough memory to level up five times in a single beacon visit.
You can do this achievement after completing Such Salt to save yourself some time.
5. Roguelike
All you need to do is to get 20 backstabs on enemies.
There are a few easy enemies to backstab such as the normal NPC mobs, the haunted knight armors, or the fishmen. You can strafe around these enemies to make it easier to get behind them.
6. Ready? Fight!
You need to riposte enemies 20 times to get this achievement.
Before doing so, you'll need to get a shield that has the DEFLECT ability.
Once you have it, you'll want to parry an enemy before you can riposte them.

How to Parry & Riposte:
  • Have your shield with deflect equipped in your left hand.
  • Wait for an enemy to start an attacking animation.
  • When it looks like the enemy is just about to make contact with you, activate the deflect.
    (Left Trigger for controllers, G for keyboard & mouse)
  • If done correctly, the enemy will be staggered. Go up to them and attack.
  • A riposte animation should play in which you thrust your weapon through the enemy.

This will take some practicing to get the timing down.
I recommend doing this to easier enemies in the early levels.
7. The Block Button
You need to perform 20 rebuffs in order to earn this achievement.
I personally found this one to be easier than the riposte one but let us get started.
First, you must obtain a shield with the REBUFF ability.
Now that you've gotten a shield with rebuff, you need to know what a rebuff is.

Parrying rewards a risky playstyle by leaving players vulnerable to getting hit but also giving them a chance to stun and do critical damage to enemies.

Rebuffing, on the other hand, rewards a defensive playstyle. It encourages players to keep blocking attacks until there is an opportunity to strike. Think of the sword-and-board play style.
You block for a bit then counterattack with decent timing.

How to Rebuff:
  • Have a shield with rebuff but KEEP BLOCKING with it.
  • Wait for an enemy to attack you.
  • When they're about to hit you, LET GO OF BLOCKING.
  • If done correctly, there'll be a shockwave from your shield clashing with the enemy.
    The enemy will be staggered and you should regain some stamina.

You'll notice that the timing is quite similar to parrying. However unlike parrying, you can do this on the same enemy multiple times if you choose not to attack them after staggering them.
8. Master's Sword
For this achievement, you need to fully upgrade a weapon to +5.
You must unlock the blacksmith by restoring the Smith's Hammer at the Oreshaft Village.

You also need these materials and at least 7750 memory to upgrade at the blacksmith:
  • 4 Slivers of Tradition
  • 6 Fragments of Tradition
  • 2 Pieces of Tradition
9. Blessing of Pain*
For this achievement, you need to level up at the Reservoir of Pain.

This is MISSABLE, make sure to do this before going into the Keep of the Stolen Crown.
You should do this during your Laid Bare and The Unwanted Savior run.

Please refer to the Innocent's Questline in Tay's guide but I will provide more context.

You'll want to progess the questline to the point where you're tasked with giving the Innocent NPC an item from the Crone. TAKE THE TUNNELS underneath the Warlock's Wilds and follow the route that leads back to Oreshaft Village. The Reservoir of Pain should be blocking the elevator.

Talk to him and he'll give you his gift to give to the Innocent. Make your way to the Innocent and GIVE HIM THE RESERVOIR'S GIFT. He'll be sent to a lovely farm upstate.

Return to the Reservoir of Pain and he will give you the option to become a martyr, accept it.
You will have unlocked the Martyrdom mode which gives you penalties the more you unburden items.
It's basically there if you wish to make the game harder for yourself BUT you will be able to level up at the Reservoir like you would with a beacon.

***You can always undo the Martyrdom Penalties by leveling up at a normal beacon but you might as well get all the penalties for another achievement.
10. Speed Run
You need to beat the game under 5 hours in a playthrough to get this achievement.
This means that you just need to get an ending, so don't worry about getting everything.
I recommend that you do this after your first playthrough since you should be more familiar with the game's routes in your subsequent games. I also recommend that you go straight to the Claim Throne ending after beating the Pipelayer for the quickest end.

Overall, this achievement is not as bad as it seems. You'll get better at playing through the game as you continue.
11. PvE Mode*
For this achievement, you must beat ALL the game's bosses without summoning Players/NPCs.
Notice how I emphasized the ALL?
Yes, you need to beat the game's 7 normal bosses and the other 2 secret bosses.

This means that you will need to engage in The Innocent's Questline and finish it.
Tay's guide has the correct route(Give the Gourd) to finishing it, so I'll link to their guide.
You must give the gourd to the Innocent before going too far into the Keep of the Stolen Crown.

*Just a reminder, you can still do the NPC questlines. Just decline their help in battle when they're by the boss door. I think you can still ask them to tether you but I wouldn't risk it.
COLLECTIBLES ACHIEVEMENTS
These achievements focus on the player's ability to gather hidden items spread across the land.
12. Fully Formatted* / 13. Song of Source*
For these two achievments, you need to collect all runes and spells respectively.
This is easily missable due to the Innocent's questline if you do not give him the items before venturing too far into the Keep of the Stolen Crown.

You also need to interact with the Merchant at the Oreshaft Village by killing his attackers before killing the Grinder, I think. Be wary that you'll need to find the Easymaker keys to unlock more stock and you'll need to buy them before he goes feral around the time you fight the Anti-Heroes at the arena.

14. Lore of the Land*
You need to restore the memory of 75 relics for this achievement.

15. Laid Bare*
You need to unburden 71 relics for this achievement.
This one is a bit more lenient than Lore of the Land because you don't need to get ALL of the items.
It is due to the fact that the Reservoir of Pain will not be accessible after you beat the Pipelayer.

You still need to get almost all the relics. Just follow the Lore of the Land guide and unburden the relics instead of restoring them.

You don't need to worry about these (Credit to Rahuu):
  • Locksmith's Serrated Saw - Required for progression.
  • Smith's Hammer - Needed for upgrading.
  • Laid Pipe - "Final boss" weapon
  • Savior's Raiment - "Final boss" armor

Also credit to Freeze Knight Snowluke 98 for making me realize that it was actually 71 instead of 70 relics that needed to be unburdened.
16. The Unwanted Savior*
You need to collect all the Martyrdom Penalties in your playthrough for this achievement.
Please refer back to 9. Blessing of Pain on how to unlock the Martyrdom mode.

Anyway you'll need to unburden at least 40 relics to get all of the penalties.
You might as well should do this in a Laid Bare run.
17. Ode to a Blacksmith*
For this achievement, you need to collect every piece of armor which also include the non-relics.

I don't know whether or not you need to collect all of these non-weapons/one-off armor pieces like the Red Racer shoes or the Underwear but I'll show you what I have in my character's inventory. If getting the armor sets listed don't get the achievement then try to get the one-off pieces listed afterwards.

Keep in mind that some of these items are missable such as needing to progress the Innocent's questline or buying from the Merchant.

Armor Sets (Torsos)

From Left to Right
  • Skeleton Set - Farm the skeletons in the Warlock's Wilds
  • Null Armor Set - Starter armor
  • Stolen Wraps Set - Near the suspicious house at Oreshaft Village
  • Tunic's Set - At one of the cells near the beginning of Rehabilitation Pits
  • Amnesiac's Set - In one of the cells at the Quest Reward Vault
  • Welcome Set - Farm the dapper NPCs in Start of Sequence
  • Heart of a Hero Set - At the large door near the Heart of Nostalgia beacon
  • Forgotten Knight Set - At Oreshaft Village on a desk near a wooden bridge leading to the mines
  • Easy Target - At the Hall of Heroes
  • Easymaker Chain Set* - Purchase these from the Merchant at Oreshaft Village
  • Chainmail Bikini Set - At the luxury area in the Lair of the Adversary
  • Anti-Hero Set - Beat all the Anti-Heroes that ambush you throughout the game
  • Forgottenspace Set - At the village area in Warlock's Wilds
  • Heroism's Sew Set* - Progress Innocent quest and beat the secret boss
    After beating the statue boss again in the rematch and giving the essence to the Old Man and to the Heart of Nostalgaia, return back to the statue room where you can pick up the set in front of the new statue.

  • Primal Set - At The Depths near where you found Lady Sco and Nhero
  • Bold Blade Set* - Progress Blade's questline or murder him in his cell
  • Disgraced Set - Farm the brown guards at the Quest Reward Vault
  • Master Chef Set - At the kitchen at Quest Reward vault right before the Gnom boss arena
  • Golden Set - Farm the golden guards at Festivity Row
  • Haunted Set - Farm the haunted knights at the Keep of the Stolen Crown
  • Civilization Set - In the spiral staircase area at Keep of the Stolen Crown
  • Savior Set - Defeat Mercucio the Pipe Layer
  • Samwitch's Set(!!!) - Need a restored Irrelevant Appendage, return to the Heart of Nostalgaia area, hug the right wall of the path to Oreshaft Village until you see a handprint on the rock, interact with it to unlock a door and enter the room to get this set
  • New Order Set - Beat Brandon Merkle after fighting the Arena

One-Off Pieces
If getting all of those didn't work then here's the rest of the pieces.

  • Unlikely Lense (Helms) - At the broken street section at Start of Sequence where you need to use Flat Traversal
  • Crown of the Undone (Helms) - At the Warlock's Wilds by the grave near the Crone's Hut
  • Save Me Mask (Helms) - At the beginning of Warlock's Wilds near a campfire where you need to kill one of the enemies to get a token
  • Lost-One Visage (Helms) - Farm the scarecrow enemies at Warlock's Wilds
  • Red Racers (Legs) - Near the end of Warlock's Wilds, there will be a spider enemy and a few skeletons
  • Leonard's Lounge Shorts (Legs) - At the Depths on a broken bridge near the cave entrance to the Grinder boss

QUESTLINE/STORY ACHIEVEMENTS
These achievements reflect the player's progression in the game's story by showing which bosses they've defeated as well as advancing NPC questlines.
18. Redemption* / 19. For Honor* / 20. For the Unwanted* / 21. Destiny Rebuked*
You need to complete the questlines of Blade, Nhero, Sco, and Vengeance.

22. Mercy* / 23. Whisked Away*
For these two achievements, you need to give the Innocent either the Reservoir's Gift or the Crone's Gourd respectively. These achievements are mutually exclusive so you need to get these in different playthroughs as they're the "make or break" part of the Innocent quest.

TRY TO DO THIS BEFORE GOING INTO THE KEEP OF THE STOLEN CROWN OR YOU'LL MISS OUT ON THESE ACHIEVEMENTS.
Use Tay's questline guide to learn how to initiate the Innocent's Questline:

Please note that Tay only covers giving the Crone's Gift to the Innocent which continues the quest.

I will cover giving the Reservoir of Pain's Gift to the Innocent which will FAIL the quest.

Follow Tay's guide until you talk to the Crone at her hut in Warlock's Wilds.
Head towards the grave and kick it down to unlock a shortcut then head down the ramp on the right.
At the end of the ramp. turn to the right and you'll see a log. Go into it and you should be in the backstage area. You'll want to go through the path labeled Oreshaft Village.
Continue going down the hallway until you reach the elevator room where the Reservoir of Pain is waiting. Talk to him and he'll give you The Reservoir's Gift to give to the Innocent.
Take the elevator and go through Oreshaft Village until you see the goofy Innocent NPC.
Whichever gift you choose to give to the Innocent will affect the rest of your playthrough.
24. The Threshold / 25. The Raider / 26. The Betrayer
You need to beat The Spirit, The Grinder, and Gnom the Consumer respectively.

27. The Puritan / 28. The Heckler(SPOILER!) / 29. The Embittered
You must beat Wunderschonenburg, The Narrator, and the Anti-Heroes of the Arena respectively.

30. The Pipe Layer / 31. The Great One (ENDING)
You must defeat Mercucio and claim his throne respectively for these two achievements.
32. The Guardian*
You must defeat the Spirit of Heroism to get this achievement.

This is a secret boss that is only accessible from progressing the Innocent's Questline.
Remember to give the gourd to the Innocent before you go too far into the Keep of the Stolen Crown.

I'll save you the trouble of looking at the questline guide and tell you that you'll need to beat Mercucio and open the door back to the Old Man's beacon. Then you will need to go back to the Spirit Statue and choose to challenge it.

Be prepared for a challenge.
33. The Martyr(SPOILER!)* / 34. And End to Suffering* / 35. Endless Possibilities*
You will need to beat The Reservoir of Pain which will complete two questlines.

36. Resurrection (ENDING)
You need to finish the Innocent's questline which involves killing the Reservoir of Pain to unlock the Resurrection ending.

You no longer need to complete the Innocent's Questline to be able to interact with the Heartflow Altar. All you need to do is interact with it after beating Mercucio in order to start NG+.

Interact with the Heart of Nostalgaia to be given the option to sacrifice yourself to it.
MISC. ACHIEVEMENTS
These achievements are a little bit random but the players try to get them for fun.
37. 404
I'll be honest, I don't exactly know how to get this achievement. I got it randomly while playing.

I read somewhere that it could be related to holding down the heavy attack. Try that.

I'll update this section if anyone can confirm it.

UPDATE:
Credit to 𝓛𝖚𝖈𝖎𝖋𝖊𝖗 for this one. You need to play the game for 4 hours and 4 minutes.
Makes a lot more sense now, lol.
38. A Guide Isn't Cheating
For this achievement, you'll first need to get to the beginning of the Warlock's Wilds.

You should be in this area where you insert the first token.
Look to the left of this structure and you should see a ramp, go up it.




When you reach the top, go to the leftmost corner of the plateau and you'll see this glowing mushroom. Hit the tree next to it around twenty times to get the achievement

39. I Lit Every Torch
For this achievement, you must light up all the torches in the Rehabilitation Pits.
There should be 24 torches in total and you must have a torch equipped.
The first two torches are right next to the cage elevator at the beginning of the area.

The next three torches are near the hologram of Wunderschoenenburg and the beacon.

Two torches are to the left and right of this initially locked shortcut door.

One torch is directly to the left of this open cell door in the first floor.

Progress to the staircase area with the scarecrow enemies, there is a torch here.

Climb up the staircase area and there will be a torch at this corner of the hallway.

Exit out of the hallway and there will be a torch on the right, you should be on the second floor.

There is a torch to light next to the dungeon gate on the second floor.

One torch can be found to the right of this door that leads to the third floor.

Afterwards, you will end up in a set of prison cells with a hole in a ground. Light the torch next to it but don't drop down just yet.

Go out the door to find a torch right next to it. Light it then drop down hole near the previous torch.

Once again, you'll be in a crumbled staircase area full of miners and scarecrows. Light the torch.

There is a torch at the corner after climbing the staircase. You should be on the other side of the third floor at this point.

There should be a torch next to a conveniently placed ramp of stone rubble near where you exited.

At the top of this ramp is a torch, you should be at the fourth floor by now.

One torch is to the right of the open cell door that leads to the final stretch of the dungeon.

The last four torches are in this corridor of open cells. Be wary of the enemies that lurk here such as the scarecrows, abominations, vampiric NPCs, and the Werewolf at the end.
40. Off the Rails*
For this achievement, you need to get the train hazard to kill an enemy at Start of Sequence.

You'll need to progress through the dangerous mess that is the Start of Sequence until you reach this area. I suggest you do this while The Narrator is still alive, but I remember that the train still happens after you beat the boss until you beat the entire game.

Every couple of seconds, a train will rush towards you. Stay away from the gaps and try not to get hit by the train, otherwise you'll get knocked down and get hit for a decent chunk of health. There are ranged enemies, so it's easier to go for them as long as you don't aggro all of them.

This achievement takes a bit of patience because the train doesn't do much damage to the enemies.
So you'll need to get the enemy's health low enough while lining them up for the train.

DLC ACHIEVEMENTS
These achievements are exclusive to the Rise of Evil DLC and mainly comprise of storylike achievements and a collectible achievement.
41. That's it!?
You need to light up ALL of the torches in Easymaker's Retreat.
If you can't tell, this is referencing another achievement that needed you to light up all torches in the Rehabilitation Pits except this one requires way more.
You will need to light up 43 torches around the Easymaker's Retreat Area with an equipped torch.

Start at the Entrance
Torches #1, 2, 3
In the beginning, there are three unlit torches along the left side of the path.
Torches #4 & 5
After dealing with the Miner Knight, light these two torches.
Torches #6 & 7
You'll encounter a few Broom NPCs, two unlit torches are on the sides of the platform.
Torches #8 & 9
Go down to the left and you'll encounter a long-necked Evil NPC, don't forget to light these two.
Torch #10
Continue onward and light the torch near the wall.
Torch #11
You should see a non-hostile Luivanni fragment, go pass him and light the torch near the ramp.
Torch #12
Go up and watch out for some Broom NPCs. The torch will be by a sign and chair.
Torch #13
Go down the ramp near Torch #12 and light the nearby torch.
Torch #14
Keep going down and light the torch on the left.
Torch #15 & 16
Descend further down while lighting these two torches on your way.
Torch #17
You'll be in a small cave, there is a torch here.
Torch #18 & 19
Continue until you are out of the cave. There are two unlit torches here.
Torch #20
Return to Torch #14 and go to the left. There are quite a few Broom NPCs here.
Go to the upward ramp and light the torch up here first.

Torch #21
Turn around and go down the ramp towards the first left path. Keep going left for a bit and you should see this edge soon. Light the torch here overlooking the paradise sign.
Torch #22 & 23
Return to Torch #12 and go up the ramp. Be wary of the Miner Knight waiting near the top.
Torch #24
Do not walk onto the broken edge just yet. Go towards the left corner that has a Broom NPC and an Evil long-neck. There is a Torch and a Chunk of Tradition nearby.
Torch #25
Return near the broken edge and walk onto it. Watch out for Brooms and Miner Knights.
There is an unlit torch on this bridge you should light before dropping down onto the next section.

Shortcut Unlock
Torch #26
By now, you should've dropped down into some tunnel area with a wandering Miner Knight.
Continue and take the first right path. It should be leading you upwards onto a pipe. There is a torch you can light before taking the ladder up to unlock a door leading back to the beacon.

Torch #27 & 28
Return back to the tunnel area and light the two torches in the corridor but do not enter the left entrance just yet.
Torch #29
Go down to the right and you should be near this exit with a torch next to it.
Torch #30
Go through the exit and down the rickety ramp until you see this torch. Return upstairs once you're done.
Torch #31
You should be by Torches #27 & 28 once again. Go through this exit but watch out for the Miner Knights. There is a torch by the ramp but don't go up yet.
Torch #32
Go to the right where you'll see another Miner Knight. Deal with him before lighting up the torch here.
Torch #33 & 34
Ahead of Torch #32 is an Evil long-neck, kill it before proceeding up. Watch out as there is another Miner Knight and a Broom NPC here. Light these two torches once you're safe
Torch #35 & 36
Return to Torch #31 and go up the ramp where you'll see these two torches.
Torch #37 & 38
There are enemies at the top of the ramp. Deal with them then light the torches on the edge.
Torch #39
You'll notice a bridge here, go ahead and light the torch.
Torch #40
Enter the cave and there should be a torch here.
Torch #41
Keep going through the cave tunnel until you see the exit with a torch right next to it.
Torch #42 & 43
You should be on a wooden path with Broom NPCs patrolling it. There are two unlit torches on the right side of the path. Light this up and you should get an achievement.
42. The Idealogue
You need to defeat Eve in the Builder's Sanctuary for this achievement.

You can access this area through the Warlock's Wilds near the beacon with the blacksmith.
The door will initially be locked with red threads and can be unlocked once you've cut down the Narrator's threads of fate.

43. The Traditionalist
You will need to defeat Luivanni's Anger in Leisure Lake for this achievement.

In order to reach Leisure Lake, you need to first cut the Narrator's threads of fate.
Next, you'll want to return to the first beacon at Oreshaft Village then go down the ramp nearby.
You should see a sewer tunnel entrance that is now accessible which you can go through to progress through all the way to Luivanni.

44. The Rise and Fall of Evil* (SPOILER)
For this achievement, you must defeat the Adversary.

This fight is MISSABLE because you'll need to have completed The Innocent's questline.

***Yeah, you cannot restore the Virtues at the unaltered Statue of Heroism, I tried it on my Unburdened run but it didn't work. The questline is manadatory for unlocking The Adversary.***

You'll need to defeat Luivanni's Anger and Eve as well as gather specific relics to unlock The Adversary.

Refer to the DLC section in this guide for more specific instructions on where to acquire these parts.

Required Relics:
  • Evil's Grip located at Oreshaft Sewer
  • Evil's Pursuit located at Easymaker's Retreat
  • Evil's Terror located at Leisure Lake
  • Evil's Gaze located at Builder's Sanctuary
  • Evil's Wrath located at Builder's Sanctuary

Steps:
1. Gather the relics listed above from both DLC areas.

2. Defeat both Eve and Luivanni's Anger to get Twisted Virtue of Progress and Twisted Virtue of Tradition respectively.

3. Return to the (Stick)Statue of Heroism where you will restore these virtues into Hatred of Future and Hatred of Past.

4. Visit the blacksmith and choose the option to Reforge. You need at least 2000 memory to reforge the Evil's Terror and Evil's Wrath with the Hatred of Future and Past respectively.

5. The reforged armor will prompt you to return it to its original master. Keep in mind that you'll lose the armor set and sword when you do this. The owner is The Old Man, he should be found at the Heartflow Altar.

6. The Adversary should be restored and will invite you to his inner sanctum.
It is the same arena that you fought and defeated Mercucio the Pipelayer in your quest.

Go meet him at the door that leads back to the Lair of the Adversary and challenge the fearsome Adversary himself in his own throne room.

45. Forensics*
For this achievement, you will need to restore all of the relics that were included in the DLC.
There should be 10 new Relics added from the Rise of Evil DLC.
You need to have completed the Innocent's questline in order to restore two Relics.

Whew, that took me quite a bit to update both this guide and the relic guide with the new DLC content. It felt fun again to explore and solve the new content which felt pretty fitting to tying up the loose ends of the game's plot.

The Last Hero of Nostalgaia is truly one of my favorite games and I'd like to thank OverTheMoon and Coatsink for bringing a cool take on the soulslike genere.

I'm adding a link to my save file[drive.google.com] to help save time for some players. Just keep in mind that this save file was made before the NG+ update and remember to back up your saves. IF you're looking at the file, you should be on the lookout for the save with the "name" variable as "Guide Me", as for the rest I do not remember besides "Kirbo" which was an Unburdened run.

few details I learned:
1: You can get into a rematch with the very first boss *before* fighting the final Great One (I just did so in one of my playthroughs)
2: The Reservoir actually appears in more than one place to give their gift. I found them in the snowy spiral area where Blade's Withered Icon was.

What not worked for me was the "Song of source" Achievment in the ng+, that one with the runes worked. And now in a fresh run for laid bare i killed blade the bold and i am sure it showed me that i get his Armor set but by unburden some stuff it wasn't in my inventory...tried 2 times then before i kill Gnom to meet him again in the Cell but nothing... so i need a second try but i hope the Song of Source is working this time. And the torches in the Rehabilitation area wasn't working in the ng+

Oh sry, i finished the next ng+ now and u have not only to give the essence to the old man after u finished talking to him u have to put the essence to the heart o Nostalgaia and after that the Armor set appears at the Statue.

Hey there it's me again. On playstation i can do achievments twice. On time for ps4 and another one for ps5 if a ps5 Version is available....now i did the same failure like i did in the first run. IT's about the "Ode to a blacksmith" and innocent trophies. U have to talk to the old man and bring him the essence of heroism after beating the secret boss "Statue of heroism" and go back to the Statue to get the armor Set. I missed that step at the ng+ and now in ng+2 cause i did not thought about that there was an armor set...

Alright I stand corrected, you really are given the option to give back to Nostalgaia to begin NG+ without needing to do the Innocent questline. I will update the guide, thanks guys for letting me know.

I also did not complete the Innocent's questline, nor fight the Reservoir, and was still given the option to give myself back to Nostalgaia. I did complete Vengeance's questline though, just felt natural.

Incidentially, I also tried reloading the save and taking the throne, but that didn't trigger the other achievement. :(
