Over half a decade and nobody has a clue
So the game's from 2017, yet I see up to 2023 there's very little information about it. Since I have nothing better to do I'll list my experiences from 2017 to 2023.

I've swapped computers from an old piece of junk I originally played on, to a modern day good one, yet still receive 20-25 fps in the game consistently identical to my previous computer. There was issues of a memory leak many years ago but I assume that was fixed, unless that's why the performance is like this modern day? Or is the game just 20-25 fps for everyone collectively? Regardless, it still works. It even saved my progress from my original unlocks and money.
Getting what you need
I'd suggest mindlessly playing the game and failing numerous times to unlock new perks and items, such as farther hitting distance items and damage boosting items. Once you've obtained a few damage boosters, longer reaching attack items, and some health boosts, you're set for slaughter.

A pile of the most useful unlocks/purchases seem to be:

-Attack Distance
#4 Butter knife
#3 Switchblade
#2 Master Sword
#1 Golden Master Sword

-Damage boost
#4 Tiny dynamite stick thingy
#3 Lady fingers centipede looking thingy
#2 Cherry bomb
#1 Golden Cherry bomb

-Health
#4 Bandaid
#3 Package of Bandaids
#2 Med-kit
#1 Gold Med-kit

-Run Savers
#3 Hammer - Low chance to spawn random bottles of health or special when hitting buildings
#2 Sledgehammer - Slightly better chance to spawn random bottles of health or special
#1 Jackhammer - Pretty sure the best chance to spawn random bottles of health or special

-Power ups and building drops, these progress by leveling them up through the unlock card game.
#6 Brick shaped thing will give you health and special orbs at random by spamming attacks
#5 Uh oh fish stinky will instant kill any micro enemy you run by plus damage buildings as you climb
#4 Uh oh bubble stinky's can destroy an entire building when at the bottom it'll rise up and shatter
#3 Hurricane wind spawning when you punch can tear through buildings in each direction
#2 Athlete's foot will spew fire from your feet so run on the top of buildings to break most of them
#1 Green troll man breaks the middle parts of every building you walk by and hits anything there
Basic advice for successful playthroughs
Step 1 - Be prepared to spam attacks. I recommend using half of the game's attack combo followed by quick light hit spam to finish anything you're attacking (minus bosses, mainly rely on light hits).
This consists of 2 weak 1 heavy hit + 1 light 1 heavy hit + quick light hit spam to finish building segments.

Step 2 - Land those attacks in effective areas. Attempt to attack two or more segments, as well as two buildings, at once. This can be enhanced with the butter knife, or master sword perks unlocked for the run duration. You may be able to smash 6 parts of 2 buildings at the same time!

Step 3 - Who said you had to stay still to attack? While jumping, continue attacking while jumping up for extra hits. This is a necessary tactic for the 5th boss by the way, so get practicing! It's helpful to try landing on an area you'll be attacking 2+ segments of the building.

Bonus - You may not even need to use combos and could simply spam light attacks if you're lucky enough to get damage boosting perks, such as cherry bombs, lady fingers, dynamite twig things. In combination with attack range increasing perks such as butter knives, switch blades, master swords, you can shred buildings and bosses in moments.
Some bugs and odd ends
I've last seen a bug fix or news on updates in... 2017. Yeah, so good luck with these.

-The remote options menu is very appealing but very rough to work through the limited functions.

-Restarting the game from the pause menu with boosted yellow health will keep your boosted yellow health at the start of your new run, for help on hardcore or to sweat less early.

-The screen will flashbang you with a bright white box background when you destroy a car, kill a boss, or do certain other things. I'm unsure if this is intentional and over-done, or a mistake and overlooked.

-Spamming buttons when rushing to start level 2 will still occasionally mute the music of the level until it is over, or on very rare occasions just at random, it was previously thought this was fixed.

-There is zero information on how to get the Sheen.exe supposed jump-scare Achievement on any media outlet I've checked through, some lucky 3% figured it out. Is it a random chance level completion visit, or play as sheen/fatigue specifically and do something?

-The Achievement for stacking 6 power-ups at once may be broken for leveled up power-ups, or certain types of ones obtained, as I've had 9 at once stacked and did not earn the Achievement for doing only 6. This includes having 9 at once from breaking buildings, 9 at once during a lucky boss fight, and even purchasing at least 8 run booster items or temporary power-up levels. Could be a single person issue or a wide ranging issue here.

-Dying to the 5th boss in the 1st stage (tested as Sheen multiple times) crashes and leads to a menu basically saying it's missing the command on what to do next and to abort the menu, closing the game and saving your current money and unlocks. To avoid this issue simply restart the game from the pause menu instead of hitting space or enter when dead.

-Once obtaining 100% unlockables, not including cutscenes, you may no longer view your unlocks menu. At least for me, it makes the beep sound and simply stays on the main menu with the tv aesthetics overlay flashing on for a moment, so you may not watch your cutscenes again or view any status of any unlocked item. This is why I didn't bother to research every item and effect and went off pure memory when making this guide, as I only completed the game twice, as of early 2023 making this. Try to get a good clue of what does what, and you'll be okay

-The menus with statistics for your game-play are all over the place with numbers and percentages, sometimes it'll be completely accurate with how you did and other times it'll say you're 99% worse than when you died on level 1 with zero attacks done as the previous game. It's very surprising the high scores segment seems to work well though any time your run is done, since almost nobody plays, you'll be in the top 10 all the time!

-On very rare occasions you may obtain a Permanently Invincible Power-up, the heart in a box looking thing. This is meant to disappear and make you vulnerable after a short time but some games I've sat for 15 minutes soaking in bullets. It'll always go away after finishing the level but you can endlessly farm money in the current level. I'm unable to replicate it when attempting to but had it happen accidentally 3 times minimal. I recall it had to be some specific issue with the timer either never starting, or never stopping, or a multitude of other power-ups would be in use at the same time. One of the occasions I recall having the game paused about 20 seconds right before grabbing the power up and it never went away, if it could be a trigger related to that.
In conclusion
I waste my time playing games I didn't finish half a lifetime ago

I just beat Hardcore as Fatigue, it didn't give me a single Achievement for beating hardcore yet, but it gave me The Voyeur which says I viewed all cutscenes.
Yet it won't let me go into my unlocks menu, because I unlocked 100% items, I have to wait for it to mess up and think I have less than that for me to view it. Confusing.

Beat it with Sheen on Hardcore and didn't get the 1st hardcore win Achievement, if I don't start getting them as Fatigue I probably will stop here.

oh yeah absoultely, I've been trying to get as many of door purchases for it, my only last thought maybe might be that Sheen.exe could be the last cutscene but that might be a stretch

I guess we'll see how it goes, you'll probably beat me to it but I'd be more surprised if anyone else is trying lol. Let me know if the get swindled or sheen exe ones pop up somehow.
