Introduction
The goal of this guide is to provide a full and complete list of all the entries in the Raider's Notebook.
This is intended for anyone who is trying to complete the notebook and needs to know what they are still missing. The guide I wished I had when I completed mine.

Please note that there will be spoilers for those who haven't completed the story missions yet. I'd recommend saving this guide till after you have completed the story fully.
FAQs
Who is this intended for?
Completionists who have already beaten all the missions in Deep in Abyss Mode.

Is there a reward for completing the notebook?
No. The only thing you get is being able to see the art associated with the entries (other than the characters which just have a screenshot of their game model) and a brief description.

Can you tell me where to find [particular item]?
I'll try. I plan to add to the section at the end giving assistance on harder to obtain entries. Many obscure items are part of one-time missions so it will be harder to verify particulars for some. If you put the request in the comments, I will attempt to assist when I can.

Entry Assistance
Star Compass
One of the hard(er/est) to obtain items if you don't know where to get it.
The Star Compass can only be dropped from fighting the Inter. Device [Layer 5]. When you defeat it, it will leave behind a bag (similar to the Foreign Raider fights) containing 3 relics. As best I can tell, the three you recieve are randomly chosen from a pool of possible relics in the game. One of the possibile relics being the Star Compass. It took me 3~4 fights.


Inter. Device [Layer 5]
The Interference Device fight is a reoccuring encounter that is intermittently available in Layer 5. Some say it can be encountered in Sandy Ice Area 1, Sandy Ice Area 2, or Hail Jail. I personally have only encountered it in Hail Jail. Specifically, just west of the fishing spot in Hail Jail about 33%-50% of the time when I go on a Layer 5 trip there. Defeating it will yeild you the entry along with 3 (random) relics.

Sakawatari
A flying creature in Layer 1. You'll need to use your Telescope to scan it. It is commonly flying in the Twin Falls area, but you have to be close enough to get the scan. I believe I got my scan from the top of the tall Northern pillar, but it appears possible from the western most plateau as well. It can be frustrating to get since it glides in and out of range.

Amaranthine-deceptor
You've likely encountered it but didn't get the entry for it as scanning them is challenging. It is the aggressive bugs that randomly hide in the Eternal Fortune patchs located in Layer 4 and Layer 5. I recommend prepping yourself to take multiple hits from them when attempting the scan. They only really become visible once you are close enough for them to attack. I just walked in to the patch they were in, zoomed out the telescope and desperately tried to keep one in frame between getting hit.

Sorry, I unfortunately got it as I went through the game before I started my completionist mindset. For what little help it may provide, the in game description is:

"A recipe left by a legendary Black Whistle. The main ingredient only exists in the fifth layer. Eating it removes all fatigue."

I'm guessing it may have come from a side quest if it wasn't part of the normal progression. I'd say make sure there aren't any offered missions from Laffi, Habo, Ozen or the Caravan Fleet. If not that, then it may have been in the crafting missions from the guild or one of the missions you stumble across in Layer 4/5. Sorry I couldn't find anything decisive. Best of luck.

For the Patchwork Hat, though I can't verify in my save having already completed it, it appears to be the reward for Shiggy's Quests.

Mild spoiler: Over the course of the game you can talk to Shiggy and Nat in their room at the Orphanage. Shiggy begins to give quests for specific monster kills or showing him particular relics he requests. Not all of the questline can be completed until notably late in the game due to some requests requiring a trip to the lowest layer. If you didn't start the quest line until later, I believe you can still start and progress it. I know you can refresh Shiggy's conversation availability by entering and leaving the abyss, but it may require less than that. If the next quest isn't up, just stop back and talk to him as you progress. It's been a while, but some of the quests in the chain may not be revealed till you reach a rank where it can be done.

Thanks for the guide!

I was able to find and fight the Inter. Device [Layer 5] before doing the Layer 5 story missions at Ido Front and getting the White Whistle.
