Yes, on prototypes there were such problems, but not on the series production ones. There is no official report of the soviet army of autoloader injury and interviews with tank crews show the same.

I was making a reference to the early T-64s but it got fixed pretty quickly. Although it has never been reported there is story's about it happening so i dont know who to trust.

There is no report of any injury caused by an autoloader from T-64/80 or T-72
