A simple guide consolidating information on all 14 achievements in the game. It assumes that the reader doesn't mind spoilers.

Introduction
The achievements in this guide are more or less arranged in a chronological order. However, one of the ending achievements (A Little Secret) can be earned earlier on, but it is more fitting to have it positioned close to its kin, therefore it has been moved to a more appropriate place.

A note on the endings:
It is best to collect all the necessary spools of thread for A Little Secret achievement, as they will be needed in order to unlock two other ending achievements later on. I will mention this again in the description of A Little Secret. The descriptions of the latter endings assume that the reader is familiar with the former endings.
01. A Fragile Heart
A Fragile Heart Caused Catherine's heart to stop beating

Missable

This is probably the easiest achievement to unlock and can be earned after traversing just a few of the first rooms. Requires Catherine to die from depleting her health bar. Simply step in the blood puddles multiple times to cause damage to her heart. I believe the achievement will not unlock if you are insta-killed, as the cause of death must be either a heart attack from something frightening (stepping in the puddles, getting scared by other occurances) or multiple slashes/stab wounds.

02. A Simple Card Game
A Simple Card Game Solved the card puzzle in the prison

Story-related

This achievement requires you to solve the card puzzle in the Prison area. As the description suggests, it's a simple math problem. The solution is: 9-8+10x2.

03. Sun and Moon
Sun and Moon Unlocked the sun and moon safes

Missable

This achievement requires you to solve the sun and moon safes puzzle in the Recreation Quarter/Area. It can technically be missed, as only one safe (the white moon safe) needs to be opened in order to progress the story. The other one (yellow sun safe) contains a number of various pills. One set of letters corresponds to the doll in the white dress (white moon safe), while the other to the doll in the yellow dress (yellow sun safe). The solution is:
  • white moon safe - SAVEME
  • yellow sun safe - DOLLIE







04. The Two Sisters
The Two Sisters Deciphered the message of the two sisters

Story-related

This achievement requires you to solve the two sisters puzzle in the East Wing area. It is a simple letter rearrangement to form two words reflected by the poses of the two sisters. The solution is: HOPENDESPAIR



05. The Big Picture
The Big Picture Put the four puzzle pieces together

Story-related

This achievement requires you to find four pieces of a certain picture and then assemble them together. Everything you need is located in the Doll Design Lab area.
  • 1st piece - inside a safe, inside Intelligence Test Room. Hope is the liar. Solution for the puzzle is:
    1. Whisky --> Blue --> Seat 1
    2. Tequila --> Yellow --> Seat 2
    3. Vodka --> Green --> Seat 3
    4. Margarita --> White --> Seat 4

  • 2nd piece - at the end of Physical Test Room
  • 3rd piece - in a safe, inside Disposable Doll Room A
  • 4th piece - on a metal slab, inside Cleaning Room (the bear will spawn behind and initiate a chase sequence)
Once all are collected, proceed to Experiment Room and interact with the computer there, then select one of the pieces in Catherine's inventory - she will automatically "get the big picture", so to speak.

06. The Curious Cat
The Curious Cat Visited all locations shown on the map

Story-related

Just as the description suggests, this achievement should unlock once you visit the last location on the map, which will be Stockholmes' Private Research area on the 4th floor.
07. Red Beauty
Red Beauty Solved the sliding puzzle on the fourth floor

Missable

While playing as Agnes, enter the room and solve a simple sliding puzzle to unlock this achievement and obtain the Ruby Name Tag from the safe (which is needed for both secret and good endings).

08. Twisted Revelation
Twisted Revelation Uncovered the identity of the dark figure

Story-related

The identity of the Dark Figure is revealed after the final chase sequence of the game and that's when the achievement should unlock. However, for me personally, it unlocked a little bit later, along with the first ending achievement that I got.
09. A Little Secret, part 1
A Little Secret Unlocked the bad ending

This is the first ending that one can get. It requires you to collect 21 spools of thread and give them to Fi (in exchange for different kinds of pills). Upon meeting him for the first time, Catherine will automatically provide him with one spool of thread (free of charge). He will then request 20 more to be delivered. Once he receives enough, he will enter the room that he was just standing in front of (East Wing, Empty Room). Simply follow him there and after the first cutscene, proceed to re-enter the room.

The spools of thread aren't particularly difficult to find, nonetheless I will provide locations of all 26 that I could find myself. 3 of them are missable (one is found in a room that later becomes locked, and two are in the basement area: one side has a locked gate preventing Catherine from going back, the other spawns an invisible doll that insta-kills her upon entering the main area there, which could be a bug). This leaves you with 23 spools of thread that can be collected even once Catherine obtains all the keys and is ready to leave the factory.
The issues preventing the player from collecting 3 particular spools of thread, that were once missable, have been fixed. You are free to explore the entire building up until the point of no return (Catherine interacting with the main entrance while having the last key in her inventory).

Spools of thread will be listed in order of their availability.
Missable spools of thread are: #10, #14, #15.

Important note!

It is advisable to meet the requirements (collecting and giving the spools of thread to Fi) for this ending even if you are not interested in getting it, as it will be necessary in receiving other endings later on.

Spool of Thread #11 - East Wing, Storage Room (beware of the monster that spawns once you try to leave with the spool)
IMPORTANT - When coming back to the area, beware of "getting in line" with the toy soldier, as this will currently result in a gamebreaking bug - it will initiate a chase sequence with the Dark Figure (even if it has been completed already), locking Catherine up for good in an inescapable area (at least I couldn't find my way out of it). However, to circumvent this, you can first push the small table that served as the sole escape route during the aforementioned original chase sequence. All in all, it is best to just grab the spool of thread on your very first visit to Newsroom, before triggering the chase sequence. - FIXED

09. A Little Secret, part 2
Spool of Thread #17 - Basement, Storage Room (from here you can push the small table to technically try to explore the basement again)
In order to obtain this spool, you will have to first enter this room in West Wing, Main Hall. The (now open) gate requires the use of the crescent key.
West Wing, West Staircase - Grab this rope, so that you can gain access to the basement.
East Wing, general area - Use the rope here to go down the hole that you fell through previously.

10. The Nightmare Ends
The Nightmare Ends Unlocked the secret ending

This ending requires you to collect 2 name tags as Catherine:
  • Amethyst Name Tag, which can be obtained in the prison area (the specific room, which contains the tag, will require a story-related item: the crescent key. The name tag will be inside a locker, the solution to the puzzle that unlocks it is 8937)
  • Emerald Name Tag, which can be obtained from a room on the 3rd floor (this one will require another story-related key: the golden key. The name tag is held by the doll in the red dress)

Once you get to the point of playing as Agnes, 2 additional items must be collected:
  • Ruby Name Tag, which serves as a reward for completing the "Red Beauty" sliding puzzle - it's inside the safe
  • Emergency Switch, which you can find in the previously unavailable part of the room, where you find Mr. Stockholmes' body; it's inside a drawer - the combination is 5040

Apart from collecting these 4 items, Agnes must see Catherine as an ally, rather than question her mental state and intentions. This is reflected by the heart visible in the upper right corner when you play as Agnes. A heart that is mostly red means that Agnes will be protective of Catherine in the final confrontation. To ensure that, you will have to interact with a few items and choose the right options. There are enough interactions for you to turn an almost completely blue heart (which is the case if you had failed to help Agnes out as Catherine during the bear chase scene) into a red one, therefore only the Agnes section matters.

Once all these requirements are met, proceed downstairs as Agnes.



Amethyst Name Tag - Prison, Upper Prison, general area (as soon as you go up the stairs from the Lower Prison)
Letters correspond to adequate prisoner's room containing the relevant sign. The solution is 8937.

1. Caplock pistol. Interactions:
  • I should take the pistol - negative
  • I should leave it be - positive

2. Statue of Catherine's mother. Interactions:
  • She reminds me of you - positive
  • She is similar to her father - negative

3. Surveillance footage. Interactions:
  • She had no other choice - positive
  • She is selfish - negative

4. Notes on the experiment. Interactions:
  • The young miss suffered so much - positive
  • The young miss' mind is not stable - negative

5. Maid doll. Interactions:
  • Lack of interaction - positive
  • Maids are inferior - negative
11. A Fitting End
A Fitting End Unlocked the good ending

This ending requires the most conditions to be met. It is basically an amalgamation of the things you have to do in order to obtain the previous two endings. Those are:
  • collecting 21 Spools of Thread and giving them to Fi as Catherine
  • collecting Amethyst Name Tag and Emerald Name Tag as Catherine
  • collecting the Ruby Name Tag and the Emergency Switch as Agnes
  • Agnes' heart must be mostly red
However, before proceeding downstairs as Agnes, you must enter the library (room located to the west of the staircase). Talk to Fi here - he will request one final spool of thread and a backpack. Both items can be found in the eastern section of the 4th floor and can be even collected prior to talking to Fi. Once Agnes possesses the items in her inventory, exhaust the dialogue with Fi (so that he physically leaves the library).

Once all these requirements are met, proceed downstairs.

12. Fleeting Hope
Fleeting Hope Unlocked the second alternate ending

The requirements for this are almost exactly the same as for "the good ending". The only difference is that you cannot collect all the items necessary for Agnes to deactivate the bear (either of the name tags or the Emergency Switch - doesn't matter).
13. All Is Lost
All Is Lost Unlocked the first alternate ending

This one is like the other alternate ending (Agnes' heart is mostly red and you are missing one of the name tags or the Emergency Switch) with the exception being that it doesn't involve interacting with Fi at all. Either don't collect 21 Spools of Thread or don't interact with Fi in the library as Agnes.
14. The Truth Revealed
The Truth Revealed Unlocked the true ending?

The only condition you need to meet in order to get this ending is to have Agnes' heart colored mostly in blue (Agnes doesn't trust Catherine and sees her as a threat). Nothing else matters.
Conclusion
I hope this guide doesn't mislead anyone and will prove useful to at least one person searching for information regarding one of the achievements. Thank you.
