When death screen comes;
- Select main menu option.
- Then select continue.
- Choose the level that you were playing.
- By doing this you will turn back to the fight you died at and death wouldn't count

No, single playthrough is not requiered.

is it necessary to complete it without any deaths in a single playthrough or can it be completed level by level individually?

It's not a game breaking bug or anything. It just resets to the checkpoint before the fight that you died at so when you get past that fight without dying game wouldn't count you as died.

How well does this work? is it consistent? Does it make you invincible or something?
