in order to get the snail follower form, you must collect 5 snail shells, and give them to 5 statues hidden around the map, once you give the last statue a shell, you'll be given the follower form

Shells drop from snails, which can be found all over Anura, I've also been told they can spawn in Darkwood but I've never seen any there, so running through Anura is generally the way to go

you'll need 5 shells for the statues, but they're fairly easy to collect. It only took me 2 runs through Anura to get all 5

There's 5 statues in total, and they can be given shells in any order

Lonely Shack
Unlocked by meeting Ratau in Darkwood
(the top middle of the screen)

Pilgrim's Passage
Unlocked by meeting The Fisherman in Darkwood
(far left side of the screen)

Spore Grotto
Unlocked by meeting a mushroom cultist in Anura
(mid-left of the screen)

Smuggler's Sanctuary
Unlocked by meeting Plimbo in Anchordeep
(mid-left of the screen)

Midas's Cave
Unlocked by finding an advertising sign in Silk cradle
(mid left of the screen, weird how most are on the left side lol)
