Community Day head (and skin) is currently available via Shift code 5B3BJ-XZWFW-T3KRZ-JBJTB-6WST9 as part of the Borderlands 3 promotional stuff.

(Zer0 is 53KBB-KXXRC-RZ66Z-WFJBB-WSRZ3, and Salvador is C35TB-WS6ST-TXBRK-TTTJT-JJH6H, while you're at it.)

To get Admiral Asskick, do I have to kill the enemies while they are phaselocked or kill them with phaselock damage?

Well, for example, you added the T.K Baha head, from the first Head Hunter DLC, there are 5 HH DLC´s and each one of them gives you a head (TK Baha, Watter Gobler, Marcus Mercenary Day, Moxxi wedding massacre, Son of Crawmerax).
