EPISODE 1

1. Syn
1. - Data-Disk
Right behind you at the start of the level.
1.1 - Tape 1/3
After sliding through the hole. Jump up the ledge and turn 180.
1.2 - Chip 1/3
In the secondary fire tutorial room. Up the first ledge and around right corner.
1.3 - Tape 2/3
In the same room. Jump up some containers and crates up to the beam.
1.4 - Chip 2/3
Outside the previous room. Right at the door turn left around some crates and jump down.
1.5 - Secret Armor
In the same outside ledge area. On the other end jump on the crates and do a long jump-dash around the corner.

In the skating arena. Behind the arcades near the exit.

In the underground lab. After using jumppad jurn around and jump across the gap. Behind some boxes.

2. Emergence
2.1 - Chip 1/3
Right out of the first gate. On the right, in some dark alleyway.
2.2 - Secret Armor
In the same first area. In the water right in front of the first gate.
2.3 - Chip 2/3
After dealing with the first batch of enemies you'll walk upon a broken pier. Follow it to a boat in front of it.
2.4 - Tape 1/3
Right after broken pier you'll have to pass through a gap too small to walk. So naturally you slide through. Doing that you'll probably miss the tape in the nook on the left of the ramp you just slid down.
Also, maybe a stretch, but potentially this is a Duke Nukem reference - this nook with it's lighting and the nuclear symbol graffiti really looks like some DN3D level exit.
2.5 - Data-Disk
In the area right after the ramp slide/tape. On the left behind the boat.
2.6 - Chip 3/3
In the next room after picking up smgs. Under the stairs.
2.7 - Teratek Key
So far it seems like when the tutorial popup said "nearby" it meant NEARBY. Barely hidden but I'm going to include them anyway.
Hard left right out of the door next to the teratek chest.
2.8 - Tape 2/3
In the parking garage. At the bottom, in the pool of what looks like some dangerous acid but actually isn't. Dive in and swim under the ramp.
2.9 - Tape 3/3
In the area with the blue key. Behind the remains of a building containing the key.
2.10 - Secret Armor
Underneath the ramp where you meet first daddy.

3. Battle Alley
3.1 - Secret Armor
Did you know you can shotgun jump Daikatana style (but way better)? That's where I figured it out. And now you know too. Use shotgun alt fire to jump to the ledge above the first "room". Jumping doesn't require charging all 3 shots, 1 is plenty.
Also this
3.2 - Chip 1/3
After clearing first arena. Go up the ramp, take right up another ramp, look across the gap to find first chip.
3.2.2 - Data-Disk
If you picked up previous chip then you probably picked up this disk. It's on the ledge you jump from to get the chip.
3.3 - Secret Health
In the water next to first arena, by the left wall.
3.4 - Secret Weapon
A bit later you'll pass by some enticingly green glowing grates. Down the grate you can find a minigun. Door is right next to it under the ramp. To open it look at a pillar on the opposite wall to the door.

3.5 - Secret-ish Armor
Behind a ship wreck in the middle of an area with jumppads.
3.6 - Tape 1/3
After jumping on the first pad look down the hole in the platform you landed on.
3.7 - Chip 2/3
Higher up, next to a splice machine you can find a small window, jump through onto a platform across.
3.8 - Secret-ish Armor
Hard to miss. In the narrow-corridor-ledge area you pass through behind a fence. Use jump pad to reach it.
3.9 - Teratek Key
After reaching the teratek crate go up the ramp and back the way you came but higher. Key is above the entrance to this wall-running tunnel.
3.10 - Secret-ish Armor
On some arches way down on the ground level. Can't be reached from the platform before 2 wall-running walls. Barely even worth the time it takes to climb back and seems to be more like a consolation prize for those who fell down.
3.11 - Tape 2/3
On the metro rails. Wait for the train to pass and quickly run the way train comes from to get the tape.
3.12 - Chip 3/3
In the last "parkour" part, after jumping on a pad and doing a wallrun you land in an area with some enemies. Turn around and you will see another "wallrun wall". Use it to get to the next platform where you find a button in an alcove. Press it and use both walls to get back to a bridge above the jumppad. There you'll find an open door.
3.13 - Tape 3/3
Another use case for the shotgun-jumping capability. Right before the door you have slide under. Use shotgun to jump to the area above that door.

4. Open Season
4.1 - Tape 1/3
Through the first door. Deal with adversaries and jump onto the pile of boxes. Shotgun jump to the platform slightly above and away with a green ad on it.
4.2 - Chip 1/3
In front of the cinema jump into the water on left side, find a yellow lit tunnel and follow it to a secret cache.
4.3 - Tape 2/3
Follow the level until you get to the second floor of the pink alley. Instead of taking the jump pad go around it down a bit, take the left turn and follow the yellow lit tunnel to a ledge, right turn, jump, take the special wall and jump off of it to the alcove in the opposite wall.
4.4 - Secret-ish Armor and Ammo
After taking a jumppad by the bank, head up small stairs and instead of taking right into the building take left, up on the ramp and across to the stash.
4. - Data-Disk
On the same rooftoop, in the dead end by the ramp on the left side.
4.5 - Tape 3/3
After going through the building you'll end up on the rooftop. Climb on the thing you entered through and use the thin strip of "wall-runnable" wall to reach the tape.
4.6 - Chip 2/3
Another chip is under the bridge between previous rooftop and the PD. Just hop of the bridge and try to land somewhere.
4.7 - Chip 3/3
Last chip is right in front of the PD, around leftmost giant pillar.
4.8 - Teratek Key
Under the stairs right next to the crate.
4.9 - Secret-ish Armor
In the cinema, under the stair behind bottom aquarium.
4.10 - Powerup
Powerup up top in the bank can be reach from either side. Not sure which path is intended both are kinda clunky.
E: Yeah, intended way is definitely using jumppad, I was kinda thinking getting it before triggering the fight.

5. Dead Plaza
5.1 - Secret Health
Very early you'll come upon a body of water. Jump down and swim to the other end, behind a wall a mega health.
5.2 - Tape 1/3
Right above the entrance into lobby/water area.
5. - Data-Disk
Above the area entrance, opposite to the rocket launcher, in an alcove on the right side.
5.3 - Chip 1/3
Get to the highest platform of this area with water. On the other side of the long corridor across the water you can see rocket launcher in a hole. It's reachable with some parkouring, walljump upgrade, shotgun jump, whatever. Point is - get to it. Dispatch the enemies and at the end of this area on the other side of rocket launcher window is a chip.
5.4 - Tape 2/3
After going through the aquarium door. Get to the bottom of the spiral ramp and jump into the water, under the pathway is first tape.
5.5 - Teratek Key
Right next to the tape. Make a left turn from the tape.
5.6 - Chip 2/3
In the room with bunch of marked walls, behind a wall on the only platform in the room. Use the jump pad to hop over the wall and grab the chip.
5.7 - Secret-ish Armor
After wall-running room you'll be dropping down a few rooms. In the 3rd room (2nd drop) there's a mega armor behind a wall.
5.8 - Tape 3/3
Proceeding through the area made out of panels you'll get locked in a corridor that opens into a room (full of enemies) with a U turn into a downwards ramp. Break the panels at that U turn and check behind the container.
5.9 - Chip 3/3
In the awful nightmare corridor. Near the exit you can find (with use of explosives or flamethrower) a button which when shot opens a door in the first room of this awful place.

6. Ascension
6.1 - Tape 1/3
Right after first door. Jump down, 180, button around corner, turns on jump pad. Up you go.
6.2 - Secret Stash
Pass through the door and slide under the meatgrinder. Turn around and look under the little bridge.
6.3 - Secret-ish Health
After going through key door and sliding under a meat grinder you have a pit by your left with a drone floating in the middle. Don't kill it but use it as a platform to get mega health.
6. - Data-Disk
On the right side of the apartment complex yard.
6.4 - Teratek Key
Above the blue key. Shotgun jump up there.
6.5 - Secret Health
At the point where game gives you a...ladders tutorial? jump down the yellow edge and do 180. Hole in the floor leads to the mega health you might've noticed earlier and to a potential instant death.
6.6 - Tape 2/3
After climbing the first ladder in the giant meatgrinder room, on the side with a box that is clearly not having a great time*.

6.7 - Chip 1/3
Right after making your way through the apartment-complex-meat-grinding-facility? gauntlet. Outside path takes a left turn onto an overhang. Instead check the dead end in front of you.
6.8 - Chip 2/3
In the "Meat-Grinding-Facility Pt2". Moved into what previously was intended path. Slide into the opening underneath the central jump pad.
6.9 - Tape 3/3
After passing through a "Syn-gore" apartment and getting outside. Go left around the building, down a level, look for a piece of rubble sticking outside, walk on it, 180, into the hole. Picking up the tape spawns some stuff. Fun fact - killing drone while standing on it sends you flying upwards.
6.10 - Chip 3/3
After taking the elevator. On a corner of the roof. Facing tower in the middle it will be back right one.

7. Rooftops
7.0 - Secret Health
At the start of the level turn around, up the slope, break through the boards and take a right for a mega health.
7.1 - Tape 1/3
At the end of the initial climb to the titular rooftops. You'll do a jump from a ladder into a wall-run into a jump pad which leads into a ladder around a left corner. Climb the ladder a bit higher and look back, jump onto the boomstick ad ledge.
7. - Data-Disk
Behind some boxes next to the jump pad on the roof.
7.2 - Secret-ish Health
When get into the room that looks like some kind of storage. Exit to the second landing-pad-thing and turn right, it's in an alcove in the wall.
7.3 - Teratek Key
From the teratek box follow the intended path a little bit, down the ramp into the room with one electric monster, look down the edge with medkits. To get back up go around the corner.
7.4 - Chip 1/3
One room later you'll be back in the previous room but in the small fenced corridor above. Do 180 on the jump pad and grab the chip above it.
7.5 - Secret-ish Armor
Mega armor behind some concrete stuff on a rooftop next to the next jump pad.
7.6 - Chip 2/3
Waaaaay, later. Night-club tower. After you destroy the scrambler check at the bottom.
7.7 - Tape 2/3
On top of the same tower, on the giant loop. (Fun fact - you can pick it up without leaving the car)
7.8 - Tape 3/3
On top of the highest tower, in the back.
7.9 - Chip 3/3
Same tower, in a bottom corner of the "traffic" room. Purple force field kills.
7. - Data-Disk
After the car park descend. On the opposite side of the room from the exit door.

8. Artifact-Zero
8.1 - Tape 1/3
After jumping section, in the very beginning of the train. Jump the gap to the second cart, do a U-turn around blue container.
8.2 - Chip 1/3
In the first cart, behind blue container on the right.
8.3 - Chip 2/3
After you defeat a big enemy and a cutscene of a hatch opening plays. Jump down, follow the path, right before you have just onto a "wallrunnable" wall, on the left on a tiny ledge behind a shipping crate. Nigh impossible to miss.
8.4 - Tape 2/3
On top of the penultimate (one before the last one) train cart, after the big battle.
8.5 - Chip 3/3
After the last big battle on the train, on top of the train engine/last cart.
8.6 - Tape 3/3
After the "of course this happens" get around or climb on top of the train cart behind you and get inside.

EPISODE 2
====================
1. Paradise Lost
1.1 - Tape 1/3
In the very first area, underneath the starting platform.
1.2 - Chip 1/3
After getting through sewer-ish area you'll end up on a ledge, some enemies indicate that you need to jump into the building below. Obviously you don't do that and you jump on top of that building. Chip in behind the vent cluster, under the train wreck.
1. - Data-Disk
In the half destroyed building with the chip on the roof. In the right side room of the top floor.
1.3 - Chip 2/3
After the long elevator ride, walk out of the building and platform up to the ledge, do 180, chip is behind and above the hole you left the building through.
1.4 - Tape 2/3
At the same ledge, jump onto the platform that has/had "zen chunk" on it and from there go around the wall in front of you, tape is behind it.
1.5 - Tape 3/3
Go through the building, after the small "arena" fight you'll have to pass under some arches. But why would you do that when you can go above them? Get on top and you won't miss the tape. Awkwardly double+wall jump to it or just use shotgun.
1. - Data-Disk
After last tape you'll have to climb a couple ladders. You'll be able to hear the disk at the end of the last ladder - it's around the corner.
1.6 - Teratek Key
Unmissiable. Take care of the crab-zombie and right after walking through the pass he was in front of you will see the key in front and above. Chest is right under the key.
1.7 - Chip 3/3
Getting to the last secret takes a worrying amount of time, so don't worry.
It's after you get the blue key. Jump down the hole behind the first blue door you found and end up at a two-way underwater fork. It's actually a three-way fork. On the left, behind some seaweed, is a small hole.

2. Toxin Refinery
2. - Data-Disk
After the cutscene. Deal with the welcoming committee and check the left committee-closet.
2.1 - Tape 1/3
After introduction to shield shotguners. When you take a jump-pad from one platform onto another do a 180, on the platform above the jump-pad.
2.2 - Tape 2/3
In the room with the blue key, under the entrance ramp.
2. - Data-Disk
After passing getting the blue key. Behind the blue key door in the same room as the key, on the left side of the hole you are supposed jump down into.
2.3 - Chip 1/3
After you go through the blue door in the blue key room and end up back in the area with the blue key door, go through the blue key door and as soon as you are outside jump on the containers, do 180, chip is on the left of the door you just passed through.
2.4 - Tape 3/3
In the same area. Instead of jumping the gap take left and walk along the path. At end a bunch of rockets and tape behind some boxes.
2.5 - Chip 2/3
After the mess that is the narrow bridge fight. On the left side there's a pile of crates, jump on top, turn around.
2.6 - Chip 3/3
In the next room, under one of the doors.
2.7 - Teratek Key
Press the button to open door, through the door you'll end up in a room with a bunch of supplies and a chest. Key is under the platform.

3. Teratek Factory
3.1 - Chip 1/3
When you take a jump-pad to C-shaped catwalks with some turrets and a robo-worm thing. After you deal with enemies check under the blue key locked door to the left, on its' right side.
3.2 - Tape 1/3
Behind the aforementioned blue key door. Near the button you have to press there's an inconspicuous box.
3.3 - Tape 2/3
At the end of the room with double sided triple horizontal mashers where's a set of them going nuts. Behind that. And if you already forgot, like I did - we got bullet-time in the previous level.
3.4 - Zhen Chunk\Data-Disk
The ability you just acquired allows you to gain momentum in any direction. Grapple at the very first hook at a right angle and send yourself flying towards a balcony to the left of that hook for some bonus cash...or just do it from the next ledge and second hook which seems a bit easier I'm overthinking things a bit
3.5 - Teratek Key
After ascending back to lobby. Following the usual rule of "key is right next to the chest". It's nigh impossible to miss as hooks are highlighted. Stand in front of the front desk and look up.
3.6 - Mega Health and Armor
From the chest take the left corridor again. Stop before the first door and hlook up.
3.7 - Tape 3/3
Now follow through that door, we have unfinished business there. In the very next room look outside - there's a hook. Look up - there's a grate through which you can see tape. Add two and two.
3.8 - Chip 2/3
Continue on that begging of the level path til you get to the room with a red laser grind and a chip behind it that you probably noticed earlier. Look up above the entrance - hook. Get up and jump dash into the gap in the laser grid.
3.9 - Chip 3/3
Backtrack a little to the 3 way split in the corridor. One way leads to a blue key door, up above in the room is an alcove with a hook.
3. - Data-Disk
After passing the red door. Right as you enter the next room, on the left behind some boxes.

4. Outskirts
4. - Data-Disk
Rather hard to miss. Early in the level you take a jump pad into an apartment building. In the square room before a wooden bridge into the next building.
4.1 - Tape 1/3
After first Teddy encounter you'll walk out onto a plank. Two hooks lead to the left. Up above the entrance is an alcove. Use first hook to reach it.
4.2 - Chip 1/3
Follow two hooks. On the corner platform there's a wooden boards wall. Behind it.
4.3 - Chip 2/3
Follow the progression path until you pass through breakable wood. A bit forward and to the right, in the gap on top of some concrete thing.
4.4 - Tape 2/3
After previous chip you drop down into an arena with pillar in the middle and water under the floor. In the water, under the pillar.
4.5 - Teratek Key
On the third level of this same arena, near vending machine. Chest is on top level.
4.6 - Chip 3/3
After you pass through the door that you open by shooting a button. When you come upon a caution marked ledge with a hook on the other side of the chasm. Around the left corner.
4.7 - Tape 3/3
Right next to the sniper rifle.
4. - Data-Disk
Just a bit before the tele-fragger. When you hop off a tiny ledge, check your right.

5. Exodus
5.1 - Tape 1/3
On the roof of the building you start inside of.
5.2 - Tape 2/3
A bit deeper into the level, in the city section. Clear out the first big group of enemies so the force field wall turns off. On the junction take left and then check the far left corner of this dead end.
5.3 - Chip 1/3
I hope whoevers placed this one name is Richard cause that's only appropriate. In one of the non-descript slum looking areas. In between you taking a mad dash through a highway and the tunnel you have to open on foot. In a corner of a turn behind some shipping containers.
5.4 - Tape 3/3
After opening tunnel doors you'll pass through a canal-looking area. At the end of that area is a ledge that you are meant to jump down. Take a moment to check on the right side of that ledge before taking the jump.
5.5 - Data-Disk
In the sever section. In front of the round concrete pipe you have to drive into.
5.6 - Teratek key
When you get locked in a tiny area with a bunch of vending machines. In the next room there's a pool of water, dive in and check under the room entrance, there's a hole with the key in it.

5.7 - Chip 2/3
The docks area end with a jump pad you take to hop over a wall. On the left right after that wall.
5.8 - Data-Disk
After jumping that wall you end up on a wide road. And early on, on your right hand side is a small building. On the side of the building that faces the lake.
5.9 - Chip 3/3
Another wall with jump pads in front it. Take the jump and notice the shore/beach kinda thing going on the right. Follow it to the tunnel hiding the chip.

6. The Wastes
6.1 - Chip 1/3
Jump on the ledge with 3 sentry-turrets, pass through the hole and take left.
6.2 - Chip 2/3
After passing through green key door and then another door. From the entrance on the right, between two ledges.
6.3 - Chip 3/3
In the same room. Walk up to the ledge above acid pit. In the distance on the left you can see a slanted support structure with another one behind it. Between those two is the last chip.
6.4 - Teratek Key
Hidden in the same manner as previous chip, in the same pit. It's in the structure around the left corner where you supposed to go. A bit before and under the hook above the acid. Chest is in the room on the right side of the platform area, right in front of the chip 2.
6.5 - Tape 1/3
Make your way to the next platform with a bunch of enemies and crab robot. Clear it out and look down the pit. Tape is behind a metal bar pillar.
6. - Data-Disk
Next to the game's BFG.
6.6 - Tape 2/3
Down in the "shoot the chains" cave. Look over the edge of the platform, behind the right rock.
6.7 - Tape 3/3
Pass through the door that opens after you shoot chains and instantly drop down under the ramp that you are on.
6. - Data-Disk
Unmissable. Next to the button you have to press, down in the pink undergrounds.

7. Pressure Point
7.1 - Tape 1/3
At very beginning, drop down and do a hard right.
7.2 - Chip 1/3
After second enclosed catwalk, near the whole in the floor is a stack of boxes, behind it.
7.3 - Chip 2/3
After passing through blue door. As soon as you are back on the "main road" take a right around a small wall.
7. - Data-Disk
Same place, same side, on the other metal-wall-with-holes-thing. "Shhhhtreet cleaners", yeah, ok Gianni, sure.
7.4 - Teratek Key
When you walk by the chest follow same corridor to the end, key is in a corner behind pillar.
7. - Data-Disk
In the same corridor as chest and key, quite literally can not miss it.
7.5 - Chip 3/3
After you take a long elevator ride, up above, on some concrete beam.
7.6 - Tape 2/3
Atop the power plant. Behind the giant purple beams.
7.7 - Tape 3/3
At the very end of the level, around a pillar opposite to the shuttle.

8. Napalm Blitz
8.1 - Chip 1/3
Behind the crashed shuttle.
8.2 - Tape 1/3
Under the starting area parking lot platform. Walk to the edge in front of the shuttle and look over the barrier, there's a metal beam on the left, tape is on that beam.
8. - Data-Disk
8.3 - Tape 2/3
Under the walkway leading to the stadium, on the left side.
8.4 - Tape 3/3
Look left along the stadium wall, right after Johny's holo portrait there's a giant metal pillar thing. Behind it.
8.5 - Chip 2/3
On the right side from the entrance you can see smoking fire. That fire hides a chip.
8.6 - Chip 3/3
In the arena entry way there's a giant piece of concrete in the way, climb it and check the left wall. (secondary shotgun mode is your friend)

====================
1. Scrapyard
1.1 - Tape 1/3
In the first area right after you gear up. Behind remaining of a wall up on the right.
*Bonus: arcade machine for an achievement
1.2 - Chip 1/3
In the very next room, down a little slope, behind some boxes.
1.3 - Chip 2/3
In the room with a force field and a couple of turrets check under the entrance ramp.
1.4 - Teratek Key
Unmissable, you get to it bit later after passing by the chest.
1.5 - Tape 2/3
In the arena you end up in after jumping some gnarly looking fans. Up above, right in front of the hole you come out of. Also from up there you can see secret zhen chunk on a pillar.
1. - Data-Disk
On the back side of the "zhen-chunk tower".
1.6 - Chip 3/3
After you clear up the arena check behind the left ramp that leads to the exit. There's a little nook in the wall.
1.? - "Scott's Mystical Head" easter egg
On the same arena take the middle jump pad to the left corner up above the previous secret, there, on the side of a pillar is an easter egg. Thanks to RHICTKY for pointing out that it's the RoTT's "Scott's Mystical Head".

2. Infestation
2.1 - Tape 1/3
Pretty early you come upon a bottomless pit. After some climbing up look back, above the entrance to this room.
2.2 - Zhen Chunk
On top of the second tower, the one you have to sniper-port to.
2.3 - Chip 1/3
Cross the chasm of green crap. On the other side turn around and check the goo-fall on the right. Through the gap up the stream.
2.4 - Chip 2/3
Above the mega-mouth.
2. - Data-Disk
By the entrance of the big arena, on the right side.
2.5 - Teratek Key
Taking the left path from the mouth. In the big open room, chest is next to the jump pad you have to take to progress and the key is behind a pillar on the other side of the room across from the chest.
2. - Data-Disk
At the top level of the "level of water goes down with each eye" part.
2.6 - Tape 2/3
Taking the right path from the mega-mouth or at the end of the left path. In the room with an eye by the ledge. Take a jump to the right from the aforementioned ledge.
2.7 - Chip 3/3
In some ruins in front of the 4th (counting "on foot" one) heart.
2.8 - Tape 3/3
In the room with hearts 5 and 6. When the lava level drops check one of the lavafalls - it leads to a tape.

3. Night Ride
3.1 - Chip 1/3
First fight of the level brings the first secret. After you take a jump pad to starboard catwalks, clear the area and look above the door in the middle - there's a shootable switch.
3.2 - Secret-ish Armor
After some grappling you'll take a jump pad up which will lead you to a ramp. Check the pipe on the side that leads underneath the ramp for some mega-armor.
3.3 - Zhen Chunk
In the room the ramp lead to. Behind container on the left.
3.4 - Tape 1/3
In the next room with a big wide ramp. Take a dive to either side, tape is underneath the ramp.
3.5 - Teratek Key
When you first make it to the more lavish looking section of the ship. Hop off the ledge and check the left side. Chest is on the left side of the very next room.
3. - Data-Disk
Somewhere in the same room as the key.

Beyond the red door, in the room behind the hole in a wall.

This one is a multi-step sequence. Passing through a blue room you end up on some T shaped path. In the middle is a closed door. With a shootable switch above. Underneath the catwalks is a button that opens a hole in a nearby wall with another shootable switch. Do both and door opens.
3.7 - Tape 2/3
In the "look for 3 switches nearby" area. In the room with a switch behind the core, underneath the pathway.
3.8 - Chip 3/3
In the right button room. You guessed it - underneath the catwalks.
3.9 - Tape 3/3
In the part of the level looks a bit raw. The secret is rather a bit too obvious. Probably won't stay this way.
When you make it to the bottom of the massive descend. To the ledge with a jump pad to be exact. Take the jump pad to the wall-runable walls that lead to the last tape.

4. Sunset Synthetica
4.1 - Chip 1/3
At the very beginning, above the "PORK" graffiti. Or above the level entrance, if you prefer.
4.2 - Chip 2/3
In the main casino hall there's a pool with a jump pad in the back, behind the jump pad is a hole. Dive in, swim through the tunnel and take the right turn.
4.3 - Tape 1/3
Rather hard to miss. In the aquarium that breaks in a cutscene.
4. - Data-Disk
Outside the club. Before hopping the barricade notice a random hook on the right. It's positioned right in front of a random hole in the building wall. Check it out.
4.4 - Chip 3/3
Outside of the casino. Pass the wooden barricade and on the other side jump down to the ground level, check wall on the left.
4.5 - Zhen Chunk
Behind the gate you open from the mushroom casino. Under the ramp.
4.6 - Tape 2/3
In the exact same area. Look up. There's a teleport point and above it an open grate. 2+2.
4.7 - Tape 3/3
Next to the second AA turret there's a dead end with a locked square door. Look up. Shotgun jump and Agile up there.
4.8 - Teratek Key
In the same area. Opposite of the last secret, next to a container is the chest. Underneath it, on the floor with the turret, in the side of this concrete structure the chest is on, is a small hole that leads to the key.

5. Terminal Eclipse
5.1 - Tape 1/3
Early in the level you have to jump down some maintenance vent thing. Jump down, take down enemies, forward is progression path, behind is solid wall, on your left is a hole with a shootable switch. Shoot it and the wall behind will slide up (literally, it sticks out of the floor above lol)
5.2 - Chip 1/3
In the space section. Big chunk of a building right before the teleporter platform. Under the the jump pad you take to the top of this chunk a hole that leads inside the building.
5.3 - Tape 2/3
A while later, after you take a portal and walk back out into space. Don't leave the platform you walk out onto too hastily. Up, above it, on a piece of wall/ceiling is another tape.
5.4 - Chip 2/3
In the borg cube. Behind the blue door. Right above the room entrance.
5.5 - Tape 3/3
On the Vermilion flagship. At the back of the very first arena, behind a container. Mega armor as a bonus.
5.6 - Chip 3/3
After the cutscene. You can see it while riding the elevator. It's up top on the next arena.
Bonus: terminal for the achievement is in this room.

Right next to the %spoiler%.

6. Vector-4
6.1 - Zhen Chunk
On the very second platform, up top, on the left side.
6.2 - Chip 1/3
In the "Shops" area. Following the objective marker a little, right after the first fight in the area you'll come upon this ledge with water circles and tree. Look over the right side.
6.3 - Data-Disk
On the over side of the big pillar in front of the 2nd floor church door.
6.4 - Tape 1/3
Rather hard to miss. Follow the objective marker after destroying the "Shops" infestation. It leads you to a waterfall ledge. Look down a bit.
6.5 - Chip 2/3
"Corps" area. Clear out the landing and take a hike all the way around this building. Chip is on the outside ledge next to one of the elevators.
6.6 - Tape 2/3
Still in "Corps". After you destroy the local infestation. Descending back down you take a "wall-run wall" and land next to a hole in the mesh wall. Check around the corner outside before going in.
6.7 - Tape 3/3
"Garden". In the first open area with 3 buttons. Underneath the platform that the jump pad is on.
6.8 - Chip 3/3
After taking the jump pad and crossing the gap. Lower on the left of the next big chunk of land.

7.1 - Chip 1/3
RIght at the start of the mission. Check the flying platform that brought you here.
7.2 - Data-Disk
Can't miss it. You really can't.
7.3 - Tape 1/3
In the lobby, behind the right pillar.
7.4 - Chip 2/3
In the hole Maw v.2 crawled out of.
7.5 - Tape 2/3
After Maw pt.2 and some traversal. Out of the double-ladder shaft instead of taking left turn right.
7.6 - Chip 3/3
In the very same room. Behind the little ramp.
7.7 - Tape 3/3
Outside the very same room. Behind the thing objective marker points to. This level has no "fade to black when fall down" thing so don't fall all the way down and have fun climbing back up.

8. Final Purge
8.1 - Tape 1/3
Clear out first group of enemies and do a 180. On the left pipe-thing.
8.2 - Data-Disk
After purging first core you'll have to traverse a row of "wall-runnable" orange cylinders. At the end of it is a platform.
8.3 - Tape 2/3
On the other side of the orange thing closest to the data-disk platform, a bit lower.
8.4 - Chip 1/3
Make it to the solid platform ground above those cylinders, clear out the area, cross the little bridge and check behind the pillar on the left.
8.5 - Chip 2/3
Right after taking the teleporter. Behind the left wall of the arena.
8.6 - Tape 3/3
After "completing the sequence" of 4 buttons and dropping down the lean pool. Narrow tunnel takes a right turn - check the opposite side first.
8.7 - Chip 3/3
On the right side of the long catwalk connecting to the central podium.

Excellent guide, but 3.4 on Battle Alley needs to be worded better. You have to look up and shoot a button behind the pillar, but the guide only explains to look at the pillar, so it can be easy to get confused. And yes, like the comment below, there are 2 or 3 Data-Disk missing from the guide, but thankfull they don't count for the achievement.

New teratek key on Syn. Its in the last arena room that is all green. It can first be collected when you get the grapling hook
