How to increase frame rate and make the game run smooth on high detail.

Increase CPU cycles.
To increase CPU cycles, and increase frame rate, hold CTRL and tap F12. Each time you tap F12, your CPU cycles will increase by one hundred.
Decrease CPU cycles.
If you're putting too much stress on your CPU, you can lower the CPU cycles by holding CTRL and tapping F11. Doing this will decrease your CPU cycles by one hundred.

I do it with another way. In file "C:\SteamLibrary\steamapps\common\The Elder Scrolls Arena\DOSBox-0.74\arena.conf" (or another location of your installing the game) need to change line:
cycles=fixed 20000

For example:
cycles=fixed 40000
