Introduction
The aim for this guide is to provide a brief and quick understanding of each of the flaws your character can have and what you need to do to trigger them.
Info. on flaws
All flaws give you +1 perk point.
You can choose to accept or reject flaws.
Flaws cannot be removed.
You are limited to 3 flaws on normal mode, 4 on hard, and 5 on supernova.
There are permanent and situational flaws.

Acquired if the player buys too many expensive items.
"After having spent so many Bits, you're likely to offer more Bits when making a purchase."
Chrono-Lethargy
-25% Tactical Time Dilation Meter Max
Acquired if the player takes too much damage while in TTD.
"Repeatedly taking damage while in TTD has reduced the amount of time you can spend using the effect."
Farsighted
Melee Weapon Skills -10
Acquired if the player is repeatedly blinded while holding melee weapons.
Repeated eye injuries have made it harder for you to notice things close to you.
Nearsighted
Ranged Weapon Skills -10
Acquired if the player is repeatedly blinded while holding ranged weapons.
"Too many blows to the eyes have made it harder to see things far away from you."
Guilt-Ridden
Ranged Weapon Sway +100%, Ranged Weapon Spread +50%
Acquired if the player kills a high amount of human enemies.
"Killing so many people has made you uneasy when fighting humans."
Permanent Concussion
Mind Attributes -1
Acquired if the player gets hit on the head by enemies multiple times.
"Having been hit in the head one too many times, your mental faculties have been permanently affected."
Permanently Crippled
Dodge Disabled, -30% Movement Speed
Acquired if the player suffers multiple attacks at their legs.
"Your legs have been damaged too many times, and your movement speed has now been permanently lowered."
Physical Damage Weakness
+25% Physical Damage Received
Acquired if the player gets hit multiple times from an enemy's physical attacks.
"Taking too much Physical Damage has made you soft and vulderable to more Physical Damage."
Plasma Weakness
+25% Plasma Damage Received
Acquired if the player gets burnt to many times from environmental hazards or from plasma damage that causes the burn effect.
"Getting burned too many times has made you susceptible to Plasma damage."
Corrosive Weakness
+25% Corrosive Damage Received
Acquired if the player suffers from too much corrosive damage.
"Suffering too many corrosive burns has made you weak against Corrosive damage."
Partially Blind
+100% Ranged Weapon Spread
Acquired if the player repeatedly takes damage to the eyes.
"Too much damage to your eyes has permanently lowered your accuracy."
Permanently Maimed
Offense Skills -20%
Acquired if the player is repeatedly hit or shot in the arms.
"Your arms have taken way too much damage, and you're no longer as good with weapons as you used to be."
Shock Weakness
+20% Shock Damage Received
Acquired if the player suffers from too much Shock damage.
"Having been shocked too many times, you have become sensitive to Shock damage."
Paranoid
Personality Attributes -1, extra Personality Attributes -1 while inside Resricted Areas or sneaking.
Acquired if the player is caught too many times by guards while roaming in restricted areas.
"Repeatedly getting caught by guards has made you jittery, doubly so when roaming around Restricted Areas or sneaking."

Dexterity -1, Perception -1, Temperament -1 while near cliffs or high up.
Acquired if the player continuously falls from high ground and takes damage from the fall.
"After repeatedly falling from great heights, you no longer function well when the ground is far beneath you."
Cynophobia
Perception -2, Temperament -1 while being attacked by Canids.
Acquired if the player is attacked too many times by a Canid.
"Mauled too many times by Canids, you get the jitters every time one's attacking, making you less effective.
Entomophobia
Dexterity -1, Perception -1, Temperament -1 while fighting insects.
Acquired if the player is attacked too many times by insects.
"After having been swarmed by insects too many times, you're no longer your best when being attacked by them."
Herniated Disk
Body Attributes -1 when at or above 50% Carry Capacity
Acquired if the player is overburdened too often??
"Carrying too heavy a load too many times has weakened your back, making you lose that spring in your step when carrying a significant amount of weight."
Herpetophobia
Dexterity -1, Perception -1, Temperament -1 while being attacked by Mantisaur, Mantipillar, Mantiswarm or Mantiqueen.
Acquired if the player gets attacked too many times by Mantisaurs/Mantipillars.
"After too many encounters with Mantipillars and Mantisaurs, you're not quite yourself when any of the Manti-family of creatures are attacking your party."
Impediphobia
Dexterity -1, Perception -1, Temperament -1 while the player is encumbered.
Acquired if the player is encumbered too many times.
"After being encumbered one too many times, you no longer function well while encumbered."
Paranoid
Personality Attributes -1, extra Personality Attributes -1 while inside Resricted Areas or sneaking.
Acquired if the player is caught too many times by guards while in restricted areas.
"Repeatedly getting caught by guards has made you jittery, doubly so when roaming around Restricted Areas or sneaking."
Pitecophobia
Dexterity -1, Perception -1, Temperament -1 while being attacked by Primals.
Acquired if the player gets attacked too many times by Primals.
"After getting repeatedly beaten up by Primals, you no longer function when they attack you and your companions."
Raptiphobia
Dexterity -1, Perception -1, Temperament -1 while being attacked by Raptidons.
Acquired if the player gets attacked too many times by Raptidons.
"Repeated encounters with Raptidons has lessened your effectiveness when they're attacking you or your companions."
Robophobia
Dexterity -1, Perception -1, Temperament -1 when there's a robot nearby. (includes SAM)
Acquired if the player encounters or gets attacked too many times by Automechanicals.
"Automechanicals now give you the willies, and when they're attacking your party you're not as effective as you should be."
Drug Addiction
Dexterity -1, Perception -1, Temperament -1 while suffering from drug withdrawal.
Acquired if the player consumes too many drugs.
"Taking as many drugs as you have has turned to an addiction."
Food Addiction
Dexterity -1, Perception -1, Temperament -1 while suffering from food withdrawal.
Acquired if the players eats a lot.
"You can't seem to stop yourself from eating, even food that's bland or horribly flavored. Not eating often will cause withdrawals."
Smoke Addiction
Dexterity -1, Perception -1, Temperament -1 while suffering from withdrawal.
Acquired if the player uses too many nicotine consumables.
"You don't know what they put in with the tobacco, but you can't go long between smokes without suffering withdrawal symptoms."
Conclusion
These Should be all the flaws you can get in game currently. Let me know if I missed anything or if something should be added.

Fun fact: If you have robophobia before you activate SAM you're character just screams when they interact with him.

Sometimes, it ain't about the damage-- it's about the personality. SAM is in the top 3 best companions - in my opinion.

Also, thanks for the guide, working on getting the flawed hero achievement-- after that. I'll only need two more...

lol feelsbadman. In my opinion SAM is one of the weaker companions anyway so you could just swap him out. Unless you are doing a rp of course, he can be pretty funny.

I had a good heavy weapons build and took the robophobia perk, then I got the robot companion and only noticed later that he activates the flaw. I just started over

i totally agree, Green. I generally avoid flaws all together when I play and I know you can make a build around them since some perks give you more damage based on how many flaws you have etc. but i don't find it to be all that worth it.

tbh i find the flaw idea intresting, but not that good in practice, idk, mabey they could have made it so you had to pick a few before getting off the emrald vale or something, similar to how fallout new vegas made you pick perks that would be flaws in your character you would have to work around
