You can only find crowbar on survival mod don't forget.

Last stand Secret room

Last stand map 1 in this house:

The double red door in the circle is the entrance, hit it 50 times with any melee weapons and here you go..

Sacrifice - Train Car

On the opposite end of the map from the train theres a small boat next to a large one. The Crowbar is on the escape stairs on the building next to the small boat. Use the boat to reach the crowbar.

Sacrifice - Barge

After you spawn, turn right and head toward the green 18 wheeler, on the right side of it there are some garbage bags in a pile. look at the corner of the roof next to the pole and above the transformer, jump and press e. The crowbar is on the corner of that building's roof.

Sacrifice - Port

From spawn, go to the left toward the generator with the container with the chainsaw in it. Go to the far left wall with the meter looking things on it, and jump on them. Turn around and jump onto the platform. Look behind the fence and look towards the roof, the crowbar is hanging there. Jump from the platform and spam E to grab it.

No Mercy - Generator

right above the weapon stache on the bottom floor on the pipes above the bench.

No Mercy - Gas station

Go toward the Burger Tank sign, climb the green truck. The crowbar is between the Burger Tank and the electrical units above the trailer of the truck.

No Mercy - Hospital

from where you spawn, turn right down the corridor to the double doors. In that room, turn right, go through the wooden door, on top of the shelf with the defibrillator is the crowbar.

No Mercy - Rooftop

on the far right corner building from where you start, next to the helipad. You have to Bunny Hop over the broken corner to get on the building with the search light. From there its on the corner of the ledge of that perch.
You can also use adrenaline shot to get there.

Crash Course - Bridge

right in front of the bridge to the right of the red car and toll booth, its between the fence and the barrier in front of the toll booth.

Crash Course - Truck depot

in the room with the Armored Truck, on the far right between the bus and the large fence. The crowbar is on the bumper of the bus between it and the wall.

Death Toll - Drains

On the sloped pipe under the blocked off stairs, its to the left where the pipe meets the concrete.

Death Toll - Church

Stand on the piano in the church and jump while looking at the ledge above you.

Death Toll - Streets

close to where you start the event is a movie theater, movie labeled "broken heart." Go to where you buy tickets, then jump. The crowbar is on top of the booth.

Death Toll - Boathouse

From where you spawn, turn right and move forward until you run into the pier. Turn right again and walk forward until you run into a boulder. Turn left from THAT boulder towards the other boulder. Go on top of that boulder, and behind the branch of the tree is the crowbar.

Dead Air - The Crane

on the right of the crane there is a flaming barrel. The crowbar is inside the barrel.

Dead Air - Construction Site

From the spawn point, go forward to just before the door to the construction building. To the right of the door is a pillar. The crowbar is behind the pillar.

Dead Air - Terminal

From the spawn point, go forward to the opposite wall. Go into the men's bathroom. The crowbar is on one of the urinals.

Dead Air - Runaway

Go to the normal spawn location from the Campaign. It is on the walkway you jump down from.

Blood Harvest - Warehouse

From where you spawn, turn left and go up onto the upper walkways. Once up there, cross over to the farthest walkway. There is a large crate underneath the walkway. Crouch and slowly walk off the walkway onto the crate, then go behind the piping, the crowbar is between the crate and the wall.

Blood Harvest - The Bridge

From where you spawn, turn right and go through the window toward the train with the yellow ladder. Go behind the train, the crowbar is on the ground.

Blood Harvest - Farmhouse

Behind the red barn is a pile of wood, the crowbar is between the wood and the barn.

Last Stand - Junkyard

Go to the crane, then go to the right toward the red container. The crowbar is in the corner behind it.

Last Stand - Lighthouse

Go around the house into the room with the spiral staircase, follow the stairs up to where they break. Shoot out the window, the crowbar is in that window

But, i found golden crowbar in Last stand Secret room when i pressed E on the keyboard in that secret room. Not some L4D2 achievement though...
