The "School Scissors" mystery that acts as the tutorial is optional, so if you're having trouble with that, an alternate way to learn the game is to play a custom run on "skeptic" difficulty.
Navigation is pretty straightforward: Starting from the first menu of a run:
1) Click the apartment button in the upper left
2) Click the room with all the papers on the wall (this is where you'll start your mysteries)
3) While in a mystery, click the icon of the area you want to go to. You can either Investigate the area (trigger a random relevant event), or if possible, on the left side of the navigation there will be "sublocations" - these are things like the shops, police, etc. At a small DOOM cost, you can enter these sublocations to shop, heal, and other goodies.
3a) Areas that progress the mystery are circled, while in some mysteries there will be a small box that may tell you to investigate an area or perform an action: these "boxed" notes are optional, but often give you rewards along with a bonus at the end of the mystery.
as for the doctor, all i'm going to say is that there's something in his office you can click on. maybe number 26 can help you.
And for #33, how do you “borrow” his medicine?
if you're trying to get more funds, you're better off investigating the apartments, as there are a number of events that can, in one event, pay many times what the treats are likely to save you. yes there's a chance you could get hurt, but there's also a chance that you could waste several visits to the dog without finding anything worth buying.
