So truly, the whole game you are playing as the made up personality of the patient, who is THE main character but is off screen. I might come back to this and add more but i feel this is good for now
Also when looking outside the window in the patients room during Chapter 2 you can see a high rise being built, occasionally hear the sounds of construction work, and eventually see when it's finished. Might mean that considerable time passes during her treatment at the hospital (you can also see snow and the trees lose their leaves to show the passing of seasons)
I think she start to get aware of it.
Because when you die in the first levels, the grunting isn't angry. It sounds more scared and weak. When you progress in the game, the grunts gets more angry and annoyed.
