Prologue

Chased by some mysterious bandits in the Kaer Morhen valley, Geralt, stumbling on a stone, falls and passes out. He wakes up in the ancient school of Kaer Morhen, now a semi-abandoned fortress. His old friend, the witcher master Vesemir, found him in the forest. Geralt has lost his memory, and remains for a long time in the fortress, where with the help of the few remaining witchers he tries to remember his past. In Kaer Morhen at the moment there is also Triss Merigold, a charming witch, her old friend and lover.
The days pass quietly, and Geralt regains his memory very slowly, but one morning some members of the outlaw gang, known as "The Salamanders", break into the fortress. Among them are none other than the magician Azar Javed, one of the leaders of the organization, and the assassin known as "the Professor". The Salamandra are keen to appropriate the alchemical secrets contained in the castle laboratory and, after a hard battle, they get what they came for and run away.
The witchers decide to follow in the footsteps of the Salamandra, and divide the areas to be searched. Geralt touches Vizima, the capital of Temeria, a powerful nation led by King Foltest; Vesemir tells the witcher that he and the ruler are very close friends, as years ago Geralt freed the king's illegitimate daughter Adda from a curse.
Chapter 1
Geralt arrives in the countryside surrounding Vizima, where he runs into Zoltan, a dwarf friend of his, and Shani, a young and handsome girl who studies medicine and works in the popular hospital of Vizima, also his old acquaintance.
The plague broke out in Vizima, and you can only enter the city with special permits. Geralt learns that the reverend of the area, a man much loved by his fellow citizens, because with prayers and sacrifices he has kept the plague away, but also very mentally closed and very little tolerant of the "different", could help him get into city. So it is, but the man has no intention of giving the witcher the pass, as long as he does not free the countryside from a terrible plague: a barghest, or a ghost dog, nicknamed "The Beast", has been running around for some time sowing panic the night. The reverend advises Geralt to ask for explanations from Abigail, the witch of the countryside, a girl little loved by the intolerant inhabitants of the place. According to the Reverend, it was she who unleashed the Beast.
The reverend's presentiments are naturally unfounded, and Abigail claims instead that she may have the answers that Geralt seeks: Alvin, a child with supernatural powers who lives with her, seems to have a particular bond with the monster. In a trance, Alvin reveals that the Beast will live as long as the evil dwells in the hearts of the inhabitants in the countryside. Geralt begins to investigate and in fact discovers many skeletons in the closet of the most "illustrious" local citizens. As he searches for information, he learns that Alvin has been captured by some Salamandra. Geralt easily defeats the men, but when he goes to bring Alvin back to Abigail he sees an angry crowd, led by the Reverend, who wants to lynch the witch. Geralt now has two options:

Protect Abigail, discrediting the Reverend and other influential locals, revealing their murky secrets, as a result of which the Beast was born. Geralt, tame the crowd, takes away the witch, and the two fight against the Beast, killing her. But for the reverend it is not enough, and he incites the inhabitants again against Geralt and Abigail, nevertheless finding death at the hands of the witcher. Geralt takes possession of the pass he finds on the body of the reverend, and greets Abigail, who runs away from that horrible place;
Or, turn the witch over to the crowd, who burns her alive. However, the Beast appears on the spot and Geralt fights it with the help of some farmers. At the end of the fight, the reverend gives Geralt the pass for Vizima.
Whatever the choice, Geralt shows up at the gates of the city, but is taken into custody by the gate guards, because they cannot think that a witcher could have obtained permits to enter Vizima, and they take him to the prison in the Temple district from Vizima.
Chapter 2
Vincent Meis, the captain of the city Guard, offers freedom to the prisoner who will be able to bring him the head of the dangerous ladybird who haunts the sewers of Vizima. Geralt volunteers and, after deciding whether or not to accept the help of the knight of the Order Sigfreid, kills the monster and gains freedom.
Released from prison, he establishes a partnership with the private investigator Raymond, with the aim of investigating the Salamandra. In the neighborhood he meets Shani, who lives there and works at the hospital of S. Lebioda, and runs into the bard Dandelion, his best friend, of which, of course, he does not remember anything. He also knows Thaler, a shrewd receiver; Ramsmeat, head of a small gang of criminals; Kalkstein, an alchemist, whom Geralt had already met briefly in the countryside, who, with the help of the witcher, tries to access a tower that stands in the middle of the marshes south of Vizima, which is said to be full of treasures and enchanted artifacts; Vivaldi, a dwarf banker friend of Zoltan, whose business has gone bankrupt; and Declan Leuvaarden, an opulent merchant, also already met outside the walls. Just Leuvaarden asks Geralt if he is really interested in destroying the Salamandra: the obvious answer leads Declan to invite Geralt to a small reception organized by the merchant (to be held in the next chapter) during which they can talk about it.
Meanwhile, Raymond discovers that the City Guard has taken a Salamandra "small fish" into custody. Geralt heads to the hospital where he is locked up, being infected, to interrogate him, but discovers that he has already been killed by someone. The autopsy conducted by her friend Shani can lead to suspicion of several people, depending on the knowledge that Geralt has. If done correctly, it reveals that only a skilled wizard could have killed the man, and Geralt begins to suspect that it was Javed. The discovery of the body of the real Raymond in the cemetery, while in the city there is a Raymond still alive, leads Geralt to definitively think that Azar Javed has taken on the appearance of the investigator to manipulate the witcher.
Geralt, with the complicity of Kalkstein, attracts "Raymond" to the tower that stands in the center of the swamp (which in the meantime the witcher has managed to open), making him believe that inside there are formulas that are of interest to the Salamandra. Geralt enters by himself, and when he exits Raymond tries to sneak up on him, although the witcher obviously knows everything and blocks him in time. Azar reveals his true form and the Professor also comes out of a magical portal, helping the wizard to fight the witcher. The battle does not end well for Geralt: after injuring the Professor, he runs away with Javed, not before the latter has stunned the witcher, who passes out.
Chapter 3
Geralt awakens in Triss's house in the Merchant Quarter, who found him in the swamps. Geralt recovers immediately, and discovers that Alvin was brought to the hospital of S. Lebioda, since he no longer has a family. Both Triss and Shani have learned about it, and both would like to adopt it. Geralt can decide which of the two to entrust him with, knowing that the two women are very jealous of each other, and that taking him from one would alienate the friendship, and the graces, of the other.
The witcher, having dealt with the matter, remembers Declan's invitation at this point, and heads to the luxurious inn in New Narakort, where the reception is held. Declan advises him before making himself known a bit by the elite of Vizima present at the party, including Princess Adda, with whom Geralt has a relationship during the party; Velerad, the burgomaster, who rules the city in the absence of Foltest; the Count De Wett of the Order, a Nilfgaardian, whose presence in the city is a sign of the friendship between Temeria and the Empire; and meets Thaler, who turns out to be actually the head of the real spies. At this point, Declan shares with the witcher useful information to eviscerate the Salamandra, and advises Geralt to speak with Jethro, Vincent Meis' right arm, who, through an informer, reveals the location of a laboratory in the sewers where the organization produces fisstech, a powerful drug. Without too much difficulty, Geralt eliminates all the criminals in the laboratory, and learns of a new target from Declan: a meeting will be held in a secret area of the Trade Quarter. The Salamandra, as a "mysterious" witcher continues to put their spins on their wheels, intend to ask for financial support from their mysterious contact in Redania, a rival kingdom in terms of trade, Temeria.
Chapter 4
Geralt is teleported from Triss to a new region of Temeria, far from Vizima and cut off from almost all the knowledge of the previous chapters. When he arrives for the first time in Dark Waters, Geralt meets Dandelion (the bard his best friend, a character found in Chapter I). Looks like Triss teleported him to that area too. Dandelion updates the witcher on the facts that are happening in the area; Geralt thus discovers that Alvin is also there, at Alina's house, a local girl. So he goes looking for the child: Alvin seems to be doing well in his new home, except for the nightmares that constantly torment him. However, Triss wisely thought of everything and sent a package together with a bard to Dark Waters, containing a dimeritium amulet and a letter for Geralt. The amulet is used by Alvin to keep his powers under control and avoid nightmares. Remember that the beginning of the chapter can take place differently depending on the relationship in which Geralt is with Alvin, Shani or Triss. Also, if Geralt had left Alvin with Triss, the gambler's Ghost will offer a game of dice to have the child's life. If Geralt loses, the ghost becomes hostile and Geralt can kill him; however the specter cannot take Alvin away in any way.

Geralt thus entrusts the amulet to the child and tells him to always wear it. However, it seems that it has no effect and Alvin continues to have his terrible nightmares. However, as long as he remains with the witcher, Alvin is happy and follows him on many adventures that the witcher will perform in this new area.

During these trips, the two talk about various matters, including Alvin's prophetic dreams and his magical abilities. Whether he wants it or not, Geralt is involved in the child's life and feels compelled to advise him as best he can.

During the chapter Geralt will meet Berengar, a witcher who left the fortress shortly before our character arrived there. Berengar often appears during the progress of the story: he is the same witcher that the reverend speaks to Geralt (he complains about, for having disappeared before ending a job) in the mission of the first chapter, to chase the ghost dog out of the countryside. We will also find a trace of it in Chapter II, when Geralt finds a witcher broke broken in the sewers, thinking he was killed by the Salamandra.

Berengar and Geralt work together for a short time. During this period Geralt discovers from Berengar the existence of a legendary armor forged by a certain Raven and that Geralt will obtain by choosing which of the three classic paths to follow (based on this choice, the armor will be composed of different materials and will have a different aspect). At this point Berengar tells why he left Kaer Morhen: he despises witchers and holds them responsible for denying him a normal life. Later, Berengar confronts Geralt in a duel, forcing him to make a choice: kill him, or leave him alive.

If Geralt spares him, Berengar helps him in the final battle against Azar Javed, in Chapter V.
If instead he kills him in a duel, Geralt will be able to keep all Berengar's objects, including a powerful medallion, which will be useful in the clash with Javed.
At the end of the chapter, the Order of the Flaming Rose arrives in the village with the intention of making indigenous people pay for hiding elf fugitives. During the fight, Alvin uses his powers and disappears again, but this time of his own volition.
