The achievements are sorted into groups each for a different medal. The type of your medal depends on the completion of each group. For the breakdown visit:

To everyone having issues, leave the game running whilst doing toher stuff (eg: play a game, afk, whatever you want) Then go check back later to see if it worked - this happened to me

Classic maps take ages now, they must have updated min round time to 1:30, or the command to change it has changed or something.

mp_humanteam ct is apparently default. change it to mp_humanteam any. Had the same issue myself just now
