The Head is in a jar close to the exit level teleport.

An easy one, on a box.

Forgotten Subway - Station Terminal

At the top of the level on a beam.

Vanishing City - City Drainage Canal

With the naked eye.

Vanishing City - Orb Amplifier

Also with the naked eye, in the way to the final level boss.
